      目的 探讨阿尔茨海默病(AD)与血管性认知障碍(VD)患者血清超氧化物歧化酶(SOD)、同型半胱氨酸(Hcy)水平及两者水平与认知功能的相关性。方法 分析44例阿尔茨海默病患者的临床资料,列为AD组,选取43例血管性认知障碍患者作为对照组,分析两组SOD和Hcy水平的差异及与认知功能的相关性。结果 AD组患者简易智力状况检查法(MMSE)评分明显高于VD组患者,差异有统计学意义(t=2.617,P=0.011)。两组研究对象在MoCA(t=1.835,P=0.069)、ADL评分(t=0.971,P=0.334)比较上,差异无统计学意义(P>0.05);AD组患者血清中SOD水平明显高于对照组,差异有统计学意义(t=2.485,P=0.015)。AD组患者血清中Hcy水平显著低于对照组,差异有统计学意义(t=6.635,P<0.01)。两组研究对象血清CRP水平比较上,差异无统计学意义(t=0.902,P=0.369);AD组患者血清中Hcy与MMSE评分呈现显著的负相关(r=-0.795,P<0.01),VD组患者血清中Hcy与MMSE评分也呈负相关(r=-0.758,P<0.01)。两组研究对象血清中SOD与MMSE不存在明显的相关关系(r=0.211,P=0.152 ;r=0.185,P=0.241)。结论 血清中SOD及Hcy水平改变可能参与了AD和VD相关发病机制,其中Hcy可作为预测AD、VD患者发生认知损伤的参考指标。
英文摘要:
      Objective To study the correlation of cognitive function and serum superoxide dismutases(SOD),serum Hcy in patients with Alzheimer's disease or vascular dementia.Methods Forty-four cases of Alzheimer's disease were selected as AD group,43 patients with vascular dementia were collected as the control group(VD group).The correlation of cognitive function and SOD and Hcy of two groups were analyzed.Results The MMSE score of AD group was significantly higher than that of control group (t=2.617,P=0.011),but no significant difference were found between the two groups in MoCA (t=1.835,P=0.069) and ADL score (t=0.971,P=0.334).In group with AD,the SOD was obviously higher than that of the control group(t=2.485,P=0.015),the Hcy was significantly lower than the control group (t=6.635,P<0.01).The differences of serum CRP in the two groups were no statistically significant (t=0.902,P=0.369).There were negative correlation between MMSE and Hcy in AD (r=-0.795,P<0.01) and VD group (r=-0.758,P<0.01).There was no significant correlation between SOD and MMSE in the two groups (r=0.185,P=0.152,r=0.211,P=0.241).Conclusion The changes of serum SOD and Hcy may be involved in the pathogenesis of AD and VD,Hcy can be used as a reference index to predict AD and VD.
