      目的 探讨吗啡联合二羟丙茶碱临床缓解晚期肿瘤患者呼吸困难的疗效。方法 用随机数字法将晚期肿瘤呼吸困难的186例患者分为2组,每组93例。对照组给予二羟丙茶碱,观察组在此基础上加用吗啡。比较两组呼吸困难情况[视觉模拟量表(VAS)评分、Brog量表]、血气指标[呼吸次数、心率(HR)及血氧饱和度(SpO2)]和临床疗效。结果 治疗后两组VAS、Brog评分、呼吸次数、HR显著降低,且观察组VAS、Brog、呼吸次数、HR显著低于对照组(P<0.05)。治疗后两组SpO2显著升高,且观察组SpO2[(90.55±22.57)%]显著高于对照组[(79.51±19.57)%],t=3.564,P<0.05。观察组有效率为84.95%,显著高于对照组的34.41%,χ2=49.354,P<0.05。结论 吗啡联合二羟丙茶碱可有效缓晚期肿瘤患者呼吸困难,改善血气指标,治疗效果满意。

      Objective To investigate the efficacy of morphine combined diprophyllineon advanced cancer patients with dyspnea.Methods 186 advanced tumor patients withdyspnea were divided into two groups by random number method,each group 93 cases.The control group was given diprophylline,and observation group was given morphine based on control group.The dyspneaindex (VAS score,Brog scale),blood gas index (respiratory rate,SpO2,HR) and clinical efficacy of two groups were compared.Results After treatment,the VAS,Brog score,respiration rate and HR of two groups were significantly decreased (P<0.05) and VAS,Brog score,respiration rate and HR of observation group were significantly lower than those of control group (P<0.05).After treatment,the SpO2 of two groups was significantly increased (P<0.05),the SpO2 ofobservation group[(90.55±22.57)%] was significantly higher than that of control group[(79.51±19.57)%],t=3.564,P<0.05.The effective rate ofobservation group was 84.95%,which was significantly higher than that of control group (34.41%),χ2=49.354,P<0.05.Conclusion Morphine combined diprophylline could effectively relieve dyspnea of patients with advanced cancer and improve blood gas index.
