      目的观察依托咪酯乳剂在麻醉维持中对老年患者血流动力学的影响及围麻醉期不良反应的发生情况。方法拟在全身麻醉下行甲状腺次全切除术老年患者50例,ASAⅠ~Ⅱ级,年龄65~75岁,随机均分为依托咪酯组25例和异丙酚组25例。常规诱导后,术中维持两组分别给予依托咪酯或异丙酚复合瑞芬太尼0.1~0.4μg.kg-1.min-1持续泵入,据患者生命体征和BIS值调整药物输注速度。记录入室后诱导前(T0)、诱导后1 min(T1)、2 min(T2)和置入喉罩后3 min(T3)、手术切皮时(T4)、麻醉后30 min(T5)拔出喉罩时(T6)的MAP、HR、BIS,记录清醒时间和拔管时间,观察并记录术后不良反应的发生情况。结果异丙酚组患者的MAP和HR于T1、T2时显著低于T0时点,在T6时HR显著增快(P<0.05);依托咪酯组于T4时MAP升高,HR增快(P<0.05)。两组苏醒时间和拔管时间差异无统计学意义。结论依托咪酯乳剂在麻醉诱导和维持中比异丙酚具有更好的血流动力学稳定性;依托咪酯复合瑞芬太尼持续输注,未发现有严重不良反应。

      Objective To observe the hemodynamics of etomidate emulsion continuous infusion and the cases of anesthesia adverse reaction in old patients.Methods Fifty old patients underwent thyroidectomy surgery were randomly allocated into etomidate group(group E)and propofol group(group P).After induction,all patients were maintained with a combined infusion of remifentanil at 0.1-0.4 μg/kg·min,combined with etomidate or propofol,the infusion rates of propofol or etomidate were based on the bispectral index(BIS).Parameters such as blood pressure,heart rate,wakeup time,extubation time and adverse events was recorded in each group at preanesthesia(T0),1min after induction(T1),2 min after induction(T2),3 min after inserting the laryngeal mask airway(T3),Operation incision immediately(T4),30min after anesthesia(T5),and the time of pull out the laryngeal mask(T6).Results Compared with T0,the MAP and HR significantly decreased at T1,T2,the HR significantly increased at T6 in group P(P0.05).The MAP and HR significantly increased at T4 in group E(P0.05).The wakeup time and the extubation time had no difference between groups(P0.05).Conclusion Etomidate emulsion has a more stable hemodynamics than propofol when used in anesthesia induction and maintenance.Moreover,Etomidate continuous infusion does not cause severe side effects.
