| 目的 研究分析2型糖尿病合并高血压患者的心率震荡特点。方法 选取就诊的2型糖尿病患者213例,根据是否合并高血压进行分组,其中单纯2型糖尿病(A组)59例,合并高血压Ⅰ级(B组)57例,合并高血压Ⅱ级(C组)54例,合并高血压Ⅲ级(D组)43例,所有患者均行24 h动态心电图监测,统计各组患者心率震荡指标,包括震荡初始(TO)和震荡斜率(TS),以及异常TO、TS的发生率,分析各组数据间差异。结果 A组的TO(%)、TS(ms/RR)、异常TO发生率(%)、异常TS发生率(%)分别为(-0.23±0.68、7.31±2.67、15.3、11.9),B组的为(0.41±0.88、5.91±2.56、24.6、22.8),C组的为(0.83±1.02、3.96±2.01、38.9、37.0),D组的为(1.20±0.98、2.77±1.87、48.8、41.9),四组相比较,在TO、异常TO发生率和异常TS发生率方面,A组低于B组、C组、D组,B组低于C组、D组,C组低于D组,而在TS方面,A组高于B组、C组、D组,B组高于C组、D组,C组高于D组,差异均有统计学意义(P＜0.05)。结论 2型糖尿病合并高血压患者的自主神经功能紊乱较单纯2型糖尿病患者更加严重,且严重程度与高血压病等级有关。 |

| Objective To analysis the characteristics of heart rate turbulence (HRT) in patients with type2 diabetes mellitus(T2DM) accompanied by hypertension.Methods Two hundred and thirteen T2DM patients were collected and divide into two groups by with hypertension or not.59 cases T2DM (Group A),57 cases T2DM with hypertension grade 1(Group B),54 cases T2DM with hypertension grade 2(Group C),43 cases T2DM with hypertension grade 3(Group C).All subjects underwent 24 hours dynamic electrocardiogram,the data of HRT,including turbulence onset(TO),turbulence slope(TS),incidence of abnormal TO and TS.Results The incidence of TO,abnormal TO and abnormal TS in group A (-0.23±0.68,7.31±2.67,15.3,11.9) were lower than those of B,C and D groups.The incidence of group B were lower than those in group C and D.The incidence of group C were lower than thoes in group D.The incidence of TS in group A was higher than that of B,C and D groups,the incidence of group B was higher than that in group C and group D,and the incidence of group C was higher than that in group D.The differences were statistically significant (P＜0.05).Conclusion The dysfunction of heart autonomic in T2DM patients with hypertension is more serious than those patients with T2DM only,its severity is associate with the grade of hypertension. |
