      目的 探讨宫颈癌患者T淋巴细胞亚群与自然杀伤细胞(NK细胞)水平和病情的相关性。方法选取226例宫颈癌患者,根据宫颈癌临床分期分为Ⅰ期组(52例)、Ⅱ期组(63例)、Ⅲ期组(57例)及Ⅳ期组(54例)四组。另选同期健康体检者60例女性作为对照组,使用流式细胞仪检测并比较上述各组T淋巴细胞亚群(CD+3、CD+3CD+4、CD+3CD+8、CD+3CD+4/CD+3CD+8)及NK细胞(CD+16CD+56)水平。结果 Ⅱ期组、Ⅲ期组及Ⅳ期组CD+3水平明显低于对照组和Ⅰ期组(P<0.05),且随着分期的增加,CD+3水平逐渐降低(P<0.05);Ⅰ期组、Ⅱ期组、Ⅲ期组及Ⅳ期组CD+3CD+4、CD+3CD+4/CD+3CD+8水平明显低于对照组(P<0.05),且随着分期的增加,CD+3CD+4、CD+3CD+4/CD+3CD+8水平逐渐降低(P<0.05);Ⅱ期组、Ⅲ期组及Ⅳ期组CD+3CD+8水平明显高于对照组与Ⅰ期组(P<0.05),且随着分期的增加,CD+3CD+8水平逐渐升高(P<0.05)。Ⅰ期组、Ⅱ期组、Ⅲ期组及Ⅳ期组CD+16CD+56数量明显低于对照组(P<0.05),且随着分期的增加,CD+16CD+56数量逐渐降低(P<0.05)。宫颈癌患者临床分期与CD+3、CD+3CD+4、CD+3CD+4/CD+3CD+8及CD+16CD+56水平成负相关(P<0.01),与CD+3CD+8成正相关(P<0.01)。结论 T淋巴细胞亚群与NK细胞水平可反映宫颈癌患者的细胞免疫功能,宫颈癌临床分期越晚,细胞免疫功能越低。

      Objective To investigate the relationship between T lymphocyte subsets and NK cells and disease condition of patients with cervical cancer.Methods A total of 226 patients with cervical cancer in our hospital were divided into stage Ⅰ group (52 cases),stage Ⅱ group (63 cases),stage Ⅲ group (57 cases) and stage Ⅳ group (cases) according to their clinical stage,and 60 cases of healthy women in the same period were selected as control group.The T lymphocyte subsets (CD3+,CD3+CD4+,CD3+CD8+,CD3+CD4+/CD3+CD8+) and NK cells (CD16+CD56+) of each group were detected by Flow cytometry and compared.Results The CD3+ level of stage Ⅱ group,stage Ⅲ group,stage Ⅳ group were significantly lower than those of control and stage Ⅰ group(P<0.05),and CD3+ level decreased with the increase of stage(P<0.05).The CD3+CD4+,CD3+CD4+/CD3+CD8+levels of stage Ⅰ group,stage Ⅱ group,stage Ⅲ group,stage Ⅳ group were significantly lower than those of control group(P<0.05),and CD3+CD4+,CD3+CD4+/CD3+CD8+ levels decreased with the increase of stage(P<0.05).The CD3+CD8+ levels of stage Ⅱ group,stage Ⅲ group,stage Ⅳ group were significantly higher than those of control and stage Ⅰ group (P<0.05),and CD3+ level increased with the increase of stage (P<0.05).The CD16+CD56+ levels of stage Ⅰ group,stage Ⅱ group,stage Ⅲ group,stage Ⅳ group were significantly lower than those of control group (P<0.05),and CD16+CD56+ levels decreased with the increase of stage (P<0.05).The clinical stage of cervical cancer was negatively correlated with CD3+,CD3+CD4+,CD3+CD4+/CD3+CD56+ and CD56+CD56+(P<0.01),and positively correlated with CD3+CD8+(P<0.01).Conclusion T lymphocyte subsets and NK cell levels could reflect the cellular immune function of cervical cancer patients,and the clinical stage of cervical cancer is later,the cellular immune function is worse.
