| 目的 比较CT和MRI在直肠癌术前局部分期诊断正确率及诊断价值。方法 选择118例直肠癌患者入选本研究,术前分别经CT检查和MRI检查,均与术后病理结果比较准确率。结果 病理检查证实118例患者中,T1期42例,T2期49例,T3期21例,T4期6例。螺旋CT诊断正确T1分期28例,T2分期31例,T3分期15例,T4分期4例。MRI诊断正确T1分期39例,T2分期47例,T3分期19例,T4分期5例。MRI检查直肠癌T1、T2、T3分期正确率均显著高于CT(P<0.05)；两种检查方式直肠癌T4分期正确率比较,差异无统计学意义 (P>0.05)。病理活检直肠癌N0分期51例,N1分期36例,N2分期31例；螺旋CT诊断直肠癌准确N0分期34例,N1分期25例,N2分期20例；MRI检查正确N0分期48例,N1分期33例,N2分期29例；MRI检查直肠癌N0、N1、N2分期均高于螺旋CT(P<0.05)。结论 CT和MRI均可应用于直肠癌术前局部分期诊断,MRI正确率更高。 |

| Objective To compare the value and accuracyof CT and MRI for the preoperative local staging diagnosis of rectal cancer.Methods 118 cases with rectal cancer in our hospital from May January 2014 to 2016 were enrolled in the study.The preoperative local staging was performed respectively by CT and MRI,and confirmed by postoperative pathology.Results Among 118 cases confirmed by the pathological examination confirmed 118 cases,T1 42 cases,T2 49 cases,T3 21 cases,6 cases of stage T4.The right spiral CT in the diagnosis of T1 staging in 28 cases,31 cases of T2 stage,T3 stage 15 cases,T4 4 cases of stage.MRI diagnostic accuracy of T1 staging in 39 cases,47 cases of T2,9 cases of stage T3 stage,5 cases of T4 stage.MRI examination of rectal cancer T1,T2,T3 staging accuracy was significantly higher than that of CT(P<0.05).The two kinds of inspection methods at T4 staging accuracy rate showed no significant difference (P>0.05).Biopsy of N0 staging in rectal carcinoma in 51 cases,36 cases of N1 stage,N2 stage 31 cases;spiral CT diagnosis of rectal cancer N0 staging accuracy of 34 cases,25 cases of N1 staging,20 cases of N2 staging.MRI examination 48 cases of N0 stage,33 cases of N1 staging,29 cases of N2 staging.MRI examination of rectal cancer N0,N1,N2 stage were higher than that of spiral CT(P<0.05).Conclusion Both CT and MRI can be used in preoperative diagnosis of local staging for rectal cancer,and the accuracy of MRI is higher than that of CT. |
