目的 观察新风胶囊对活动期类风湿关节炎(RA)脂蛋白代谢的影响。方法 选取65例活动期RA患者按随机数字表法分为治疗组(新风胶囊组)40例,对照组(甲氨蝶呤组)25例,另设健康对照组20例。观察各组患者脂蛋白指标的变化、临床疗效、脂代谢、关节症状体征、活动性指标的变化及相关性分析。 结果 与健康对照组相比,单纯RA组及RA合并组前白蛋白(PA)、总白蛋白(TP)、白蛋白(ALB)、高密度脂蛋白(HDL)、载脂蛋白A1 (ApoA1)显著降低(P<0.05)。与RA合并组相比,单纯RA组TP、球蛋白(GLO)显著升高,载脂蛋白B(ApoB)显著降低(P<0.05)。HDL与IgM呈明显正相关,与ESR、CRP呈明显负相关(P<0.05)。ApoA1与IgM、C3呈明显正相关,与ESR、CRP、关节疼痛呈明显负相关(P<0.05)。两组总有效率相比差异无统计学意义(P>0.05),但治疗组显效率高于对照组(P<0.05)。与治疗前相比,两组治疗后均能显著降低ASO、ESR、CRP、α-AGP、RF,显著改善患者关节疼痛、关节肿胀、关节压痛和晨僵时间(P<0.05或P<0.01);但治疗组治疗前后相比PA、HDL、ApoA1、ApoB显著升高(P<0.05或P<0.01);而对照组除PA显著升高(P<0.01)外,其余均无显著变化(P>0.05);与对照组相比,治疗组PA显著增高,ESR显著降低,且在改善关节疼痛、关节肿胀、关节压痛和晨僵时间显著优于对照组(P<0.05或P<0.01)。 结论 RA患者存在脂蛋白代谢异常,新风胶囊通过调节细胞因子平衡,免疫抗炎,升高脂蛋白。 |

Objective To study the effect of Xinfeng Capsule(XFC) on lipo-protein metabolism in patients with active rheumatoid arthritis (RA).Methods Sixty-five patients with active RA were divided into the treating group with 40 patients(XFC group),the control group with 25 patients(MTX group) and the normal group (NC) including 20 healthy subjects according to the random number table.The changes in lipoprotein index,clinical efficacy,lipid metabolism,symptoms and signs of arthrosis and the changes and correlation analysis of activity index of patients in each group were observed.Results Compared with those of the NC group,the prealbumin (PA),total protein (TP),albumin (ALB),high density lipoprotein (HDL) and ApolipoproteinA1 (ApoA1) of the simple RA group and the combined RA group significantly decreased (P<0.05).Compared with those of the combined RA group,the TP and GLO of the simple RA group significantly increased (P<0.05) while ApoB significantly decreased.HDL was positively correlated with IgM,and negatively correlated with ESR and CRP(P<0.05).ApoA1 was positively correlated with IgM and C3 and negatively correlated with ESR,CRP and arthritis(P<0.05).No significant difference was shown between the general efficiency of the two groups(P>0.05),but the marked effective rate of the treatment group was higher than that of the control group(P<0.05).There was no significant difference between the pre-treatment clinical index of the two treatment groups(P>0.05);compared with the conditions before treatment,both treatments can significantly lower ASO,ESR,CRP,α-AGP and RF and significantly attenuate arthralgia,arthrocele,joint tenderness and time of morning stiffness(P<0.05 or P<0.01).Compared with the pre-treatment index,PA,HDL,ApoA1 and ApoB of the treatment group significantly increased(P<0.05 or P<0.01);but in the control group,there was no significant change in all the index(P>0.05)except a significant increase of PA(P<0.01).Compared with the control group,the treatment group showed that PA significantly increased,ESR significantly decreased and the effect in improving arthralgia,arthrocele,joint tenderness and time of morning stiffness was significantly superior to that of the control group(P<0.05 or P<0.01).Conclusion The abnormality of lipoprotein metabolism exists in RA patients.XFC can improve immunity and guard against inflammation,increase lipoprotein metabolism by modulating the balance of cytokines. |
