| 目的 探讨不同时间应用缬沙坦氢氯噻嗪片对非杓型高血压患者晨峰血压及内皮功能影响。方法 纳入74例1~2级非杓型高血压患者,用随机化编码表分为晨起服药组和睡前服药组,分别在晨起或睡前服用缬沙坦氢氯噻嗪片(每片含缬沙坦80 mg及氢氯噻嗪12.5 mg),所有病例治疗前及治疗12周后进行动态血压及内皮功能检测。结果 两组患者治疗后全天血压、血压晨峰阳性率、收缩压晨峰差值较治疗前均显著降低(P<0.01)；治疗后两组间全天血压差异无统计学意义(P>0.05),睡前服药组血压晨峰阳性率、收缩压晨峰差值、血流介导的血管舒张功能、硝酸甘油介导的血管舒张功能的改善优于晨起服药组(P<0.05)。结论 睡前服用缬沙坦氢氯噻嗪片能更好降低患者血压晨峰,改善内皮功能。 |

| Objective To explore the effects of valsartan-hydrochlorothiazide taken at different time on morning blood pressure surge and endothelial function of non-dipper hypertension.Methods Seventy-four patients with stage 1 to 2 non-dipper hypertension were randomly assigned to receive valsartan-hydrochlorothiazide (80 mg/12.5 mg) on awakening (awakening group) or at bedtime (bedtime group).Ambulatory blood pressure monitoring,Flow-mediated dialation (FMD) and Nitroglycerin Mediated Dilation(NMD) were measured before and after treatment.Results Compared with the baseline,the blood pressure,morning blood pressure surge(MBPS),FMD and NMD were dramatically improved after treatment in both two groups (P<0.01).The 24-hour systolic blood pressure (SBP)/diastolic blood pressure (DBP) were similar between the two groups after treatment (P>0.05).Compare with the awakening group,MBPS,FMD,NMD of the bedtime group were significantly improved (P<0.05).Conclusion Valsartan-hydrochlorothiazide given at bedtime could efficiently reduce the MBPS of non-dipper hypertension and improve endothelial function. |
