| 目的 观察脑电双频指数(BIS)监测应用于老年腹腔镜手术患者的临床价值。方法 选择老年腹腔镜手术患者80例,按随机数字表法分为观察组和对照组,每组40例。观察组采用BIS监测术中麻醉深度,对照组根据临床经验实施麻醉,观察两组丙泊酚用药量、苏醒时间及拔管时间,并采用简易精神状态量表(MMSE)对患者术前1天(d0)、术后1天(d1)、术后3天(d2)、术后7天(d3)进行评分。结果 观察组丙泊酚用量少于对照组(P<0.05)；观察组苏醒时间及拔管时间比对照组短(P<0.05)；与d0时比较,对照组及观察组d1、d2时MMSE评分降低,对照组下降更明显(P<0.05)；观察组术后认知功能障碍发生率(30.00%)低于对照组(55.00%),P<0.05。结论 脑电双频指数监测用于老年腹腔镜手术患者,能减少丙泊酚的用量,缩短苏醒时间及拔管时间,降低术后认知功能障碍发生率。 |

| Objective To observe the clinical value of bispectral index (BIS) monitoring in elderly patients undergoing laparoscopic surgery. Methods Eighty elderly patients with laparoscopic surgery were selected.These cases were randomly divided into the observation group and the control group(n=40 each).BIS was used to monitor the depth of anesthesia in the observation group.The control group was anesthetized according to clinical experience.Dosage of propofol,wake time and extubation time were observed in the two groups,and Mini-Mental State Examination (MMSE) was used to evaluate the result at 1 day (d0) before operation and 1 day (d1),3 day (d2),7 day (d3) after operation. Results The amount of propofol in the observation group was lower than that in the control group (P<0.05).The wake time and extubation time of the observation group were shorter than those of the control group (P<0.05).Compared with d0,The score of MMSE decreased in both two groups at d1 and d2, which decreased more significantly in the control group (P<0.05).The incidence of POCD in the observation group (30.00%) was lower than that in the control group (55.00%) (P<0.05) after operation. Conclusion Bispectral index monitoring in elderly patients undergoing laparoscopic surgery can reduce the dosage of propofol,shorten the wake time and extubation time,and improve postoperative cognitive. |
