| 目的 探讨桃红四物汤对2型糖尿病下肢血管病变患者的临床疗效及可能的机制。 方法选取66例2型糖尿病下肢血管病变患者,按随机数字表法分为研究组及对照组各33例。对照组给予降糖、调脂、扩血管等西药治疗,进行个性化治疗,治疗后空腹血糖控制在7 mmol/L,血压低于140/90 mm Hg,三酰甘油低于1.8 mmol/L,总胆固醇低于6.5 mmol/L。研究组在对照组基础上加用桃红四物汤中药治疗。观察两组踝肱指数、中医证候学积分、临床疗效、血流动力学(血浆比黏度、全血黏度等)及内皮素(ET) 水平。 结果 两组患者治疗后踝肱指数、中医证候学积分较治疗前均有不同程度改善(P<0.05),研究组在改善踝肱指数、中医证候学积分等方面优于对照组(P<0.05)；研究组临床总有效率高于对照组(P<0.05)；研究组患者的全血黏度、血浆比黏度和纤维蛋白原均低于对照组(P<0.01)；研究组患者血清ET水平下降程度优于对照组(P<0.01)。 结论 西药治疗基础上联合桃红四物汤治疗2型糖尿病合并下肢血管病变,可改善临床症状、改善血流动力学及保护血管内皮细胞。 |

| Objective To explore clinical effect of taohongsiwu decoction on vascular diseases of lower limbs of type 2 diabetes mellitus and its possible mechanism. Methods Sixty-six patients of type 2 diabetes mellitus (T2DM) with lower extremity vascular disease were selected and divided into research group and control group by random number notation.Western medicine treatments such as glycemic control,lipid-modulating and dilation of vessels were given to the control group.The research group was treated with taohongsiwu decoction based on the control group.The changes of ankle brachial index (ABI),TCM syndromes score,clinical efficacy,hemodynamic (plasma specific viscosity,whole blood viscosity,fibrinogen) and endothelin (ET) levels were observed. Results After treatment,the ABI and TCM syndrome scores in two groups were improved to different degrees compared with that before treatment (P<0.05).The research group was superior to the control group in improving ABI and TCM syndrome score (P<0.05).The clinical efficiency of the research group was higher than that of the control group (P<0.05).The whole blood viscosity,plasma viscosity and fibrinogen of the research group were lower than those of the control group.The research group was superior to the control group in decreasing ET levels (P<0.05). Conclusions Combination of taohongsiwu decoction and western medicine can improve clinical symptoms,hemodynamics and protect vascular endothelial cells in type 2 diabetes mellitus patients with lower extremity vascular disease. |
