      目的 探析冲击波治疗结合下肢肌力锻炼对膝骨关节炎疼痛的疗效。 方法 采用随机数字表法将84例膝骨关节炎疼痛患者分为对照组与联合组,每组42例。两组患者均运用冲击波治疗,以膝关节痛点为中心,冲击频率5~10 Hz,能量1.5~3.0 bar,3 000下/次,每次20 min,一日2次,连续治疗1周;联合组同时进行下肢肌力锻炼,包括直腿抬高锻炼、肌肉锻炼、牵引锻炼、站立锻炼以及步行锻炼;对比两组的临床疗效、疼痛程度(NRS评分)、骨性关节炎指数(WOMAC)评分、膝骨关节炎严重度评价指数(Lequesne-Mery)评分。 结果联合组总有效率(95.2%)高于对照组(81.0%),P<0.05;联合组治疗后NRS、WOMAC、Lequesne-Mery的得分[(1.6±0.2)分、(15.5±6.1)分、(4.8±2.1)分]皆低于对照组[(2.9±0.4)分、(22.8±7.6)分、(7.0±3.5)分),P均<0.05。 结论 冲击波治疗结合下肢肌力锻炼对膝骨关节炎疼痛有效,可减轻疼痛症状,降低膝骨关节炎严重程度。

      Objective To explore the effect of shock wave therapy combined with lower limb muscle strength exercise on knee osteoarthritis pain in order to promote patients to relieve pain quickly,improve limb function and recover health as soon as possible. Methods 84 patients with knee osteoarthritis pain were randomly divided into control group and experimental group,each group was 42 cases.The two groups were treated with shock wave.The shock frequency was 5-10 Hz,the energy was 1.5-3.0 bar,3000 times,20 minutes each time,twice a day for one week.Lower extremity muscle strength exercises,including straight leg lifting exercises,muscle exercises,traction exercises,standing exercises and walking exercises;compared the two groups of clinical efficacy,pain (NRS score),Osteoarthritis Index (WOMAC) score,knee osteoarthritis severity evaluation index (Lequesne-Mery) score. Results The total effective rate of the experimental group (95.2%) was higher than that of the control group (81.0%) (P<0.05),and the scores of NRS,WOMAC,Lequesne-Mery (1.6±0.2,5.5±6.1,4.8±2.1) in the experimental group were lower than those of the control group (2.9±0.4,2.8±7.6,7.0±3.5) after treatment (P<0.05). Conclusion Shock wave therapy combined with lower limb muscle strength exercise is effective for knee osteoarthritis pain,can greatly reduce pain symptoms,and the severity of knee osteoarthritis.
