| 目的 研究和分析内镜手术对微小甲状腺癌治疗效果的影响。方法 回顾性分析240例甲状腺微小癌患者的病历资料,按采用的手术方式不同分为A、B、C三组。A组92例患者,采用一侧甲状腺切除+峡部切除+对侧次全切除术+两侧淋巴结清除术；B组共计83例患者,采用甲状腺一侧腺叶切除+峡部切除+中央区淋巴结清除术；C组共计65例患者,采用一侧甲状腺切除+峡部切除+同侧颈淋巴结改良清除术。评价治疗前后三种手术的治疗效果,并测定各组患者血清钙、甲状旁腺激素的水平。结果 三组患者经对应的手术治疗后,A组总有效率为90.2%,B组总有效率为91.6%,C组总有效率为95.2%,C组手术方式的总有效率高于A和B组。手术后A组和C组低血钙发生情况明显高于B组。三组甲状旁腺激素(PTH)均出现不同程度的降低。结论 内镜手术治疗微小甲状腺癌是较为成熟和高效的治疗方案,常伴随术后一定程度的低血钙症的发生。 |

| Objective To study the effect of endoscopic surgery on the treatment of minimal thyroid carcinoma. Methods 240 patients with thyroid microcarcinoma treated are studied in this paper.Different surgical treatments were adopted according to the specific conditions of each patient.According to the different surgical methods,these patients were divided into three groups:A,B and C.To evaluate and determine the therapeutic effect of three kinds of surgery before and after treatment and the levels of serum calcium and parathyroid hormone in each group. Results In the three groups after the corresponding surgical treatment,the total effective rate in group A was 90.2%,the total effective rate in group B was 91.6%,the total effective rate in group C was 95.2%.The total effective rate in group C was higher than that in group A and group B.The incidence of hypocalcemia in the A group and the C group was higher than that in the B group after operation.The three groups of PTH showed different degrees of reduction. Conclusions Endoscopic surgery for the treatment of small thyroid carcinoma is a relatively mature and effective treatment.However,a certain degree of hypocalcemia is often associated with postoperative morbidity. |
