| 目的 分析不同颈椎后路单开门椎管扩大成形术治疗多节段脊髓型颈椎病的临床效果。方法 选取接受手术治疗的80例多节段脊髓型颈椎病患者,按照随机数字表法分为对照组(40例)与研究组(40例)。患者均接受颈椎后路单开门椎管扩大成形术,对照组术中采用丝线悬吊椎板,研究组术中采用微型钛板固定椎板,分析两组治疗效果。结果 两组手术时间、术中出血量、术后引流量、总有效率差异无统计学意义(P>0.05)；研究组术后并发症发生率明显低于对照组(P<0.05)；术后12个月,研究组视觉模拟评分(VAS)明显低于对照组,颈椎曲度明显大于对照组(P<0.05)。结论 颈椎后路单开门椎管扩大成形术治疗多节段脊髓型颈椎病可取得显著效果,以微型钛板固定椎板能够减轻远期术后颈椎疼痛、维持颈椎曲度。 |

| Objective To compare the clinical effects of different posterior cervical single open-door laminoplasty for multi-segment cervical spondylotic myelopathy.Methods Eighty patients with multi-segment cervical spondylotic myelopathy were randomly divided into control group (40 cases) and study group (40 cases).All patients underwent posterior cervical single open-door laminoplasty.Silk suspending lamina is applied in the control group,while micro-titanium plates was applied to fix the lamina in the study group.The therapeutic effects of two groups were compared and analyzed.Results There was no significant difference in the operation time,intraoperative blood loss,postoperative drainage volume and efficiency between two groups (P>0.05).The postoperative complication rate in the study group was significantly lower than that in the control group (P<0.05).The visual analog score (VAS) of the study group was significantly lower than that of the control group at 12 months after operation,and the cervical curvature was significantly higher than that of the control group (P<0.05).Conclusion Posterior cervical single open-door laminoplasty can achieve significant effect in the treatment of multi-segment cervical spondylotic myelopathy.Moreover,the fixation of lamina by micro-titanium plate can alleviate long-term postoperative cervical spondylosis and maintain cervical curvature,which has significant application value. |
