目的 探讨以镍钛记忆合金弓齿钉微创手术的方法治疗髌骨横行骨折,与传统克氏针张力带的方法治疗的临床效果的差异。方法 回顾分析76例髌骨横形骨折患者病历资料,分别采用镍钛记忆合金弓齿钉和克氏针张力带进行治疗,从术中情况、术后恢复和术后6个月膝关节功能方面进行比较。结果 在手术时间、出血量、切口长度、切口渗出、住院时间和术后并发症(皮肤高突)方面,弓齿钉组明显优于克氏针张力带组(P<0.05);而在骨折愈合时间和术后6个月Bostman 功能评分方面,两组差异无统计学意义(P>0.05)。此外,术后4周时起,患侧与健侧肢体的弯曲活动度差异无统计学意义。结论 髌骨横形骨折,镍钛记忆合金弓齿钉疗法组织损伤小、手术时间短、住院时间短、固定牢靠、美观性好,是一种可靠的微创疗法,优于克氏针张力带法。 |

Objective To explore the clinical outcomes between nickel-titanium arched shape memory alloy connector (hereafterreferred to as the ASC) and kirschner wire tension band in treating transverse patella fracture.Methods A total of 76 patients with transverse patellar fractures were retrospectively analyzed.The ASC and the Kirschner wire tensionband were used for treatment of transverse patellar fractures.The intraoperative conditions,postoperative recovery,and postoperative knee function at 6 months were compared.Results The operative time,blood loss,incision length,incision effusion,hospital stay and postoperative complications (hyperprotrusion of skin) were significantly better in the ASC group than in the K-wire group (P<0.05).There was no significant difference between the two groups in fracture healing time and Bostman function score 6 months postoperation (P>0.05).In addition,there was no significant difference in flexural mobility between the affected side and the healthy side after 4 weeks postoperation.Conclusion For patella transverse fracture,nickel-titanium arched shape memory alloy connector therapy can short operation time and hospital stay,firm fixation,good aesthetics,which is a reliable minimally invasive therapy with small tissue injury,generally better than kirschner wire tension band method. |
