目的 探讨原发性高血压患血压变异性(BPV)与颈动脉硬化的关系。方法 选择中老年高血压病患者共154例,同期进行24 h动态血压监测及颈动脉血管超声检查,根据颈动脉内膜中层厚度(IMT)将其分为IMT正常组(69例)及颈动脉硬化组(85例),以24 h收缩压变异性中位数为分界点,分为低 BPV 组(77例)和高 BPV 组(77 例)。结果 颈动脉硬化组较颈动脉IMT正常组相比,颈动脉硬化组24 h平均收缩压、24 h平均舒张压、24 h收缩压变异性、24 h舒张压变异性、IMT均显著增高(P<0.05);相对于低BPV组患者,高BPV组患者有更高的颈动脉IMT值及斑块检出率(P<0.05);24 h平均收缩压变异性与颈动脉IMT(r=0.64,P<0.05)呈正相关。结论 高血患者颈动脉硬化及斑块的形成与血压变异性密切相关。 |

Objective To investigate the relationship between blood pressure variability (BPV) and carotid atherosclerosis in patients with essential hypertension.Methods 154 elderly patients with hypertension treated were selected,and ambulatory blood pressure monitoring and carotid artery ultrasound examination were performed in the same period.Patients were divided into IMT(intima-media thickness) normal group (69 cases) and carotid atherosclerosis group (85 cases) according to the carotid IMT.Taking the median of 24-hour average systolic blood pressure variability (24hSBPV) as the cutoff point,patients were divided into low BPV group (77 cases) and high BPV group (77 cases).Results Compared to the normal IMT group,the 24-hour systolic blood pressure (24hSBP),24-hour diastolic blood pressure (24h DBP),24-hour SBPV,24-hour diastolic blood pressure variability(24hDBPV) and IMT in the carotid atherosclerosis group were significantly higher (P<0.05).Compared to the low BPV group,patients in the high BPV group had higher IMT value and plaque detection rate of carotid artery (P<0.05).24h SBPV was positively correlated with carotid IMT.Conclusion Carotid atherosclerosis and the plaque formation in hypertensive patients are closely related to blood pressure variability. |
