| 目的 探讨腹膜外腹腔镜疝气修补术在老年腹股沟疝中的应用价值。方法 选取老年单侧腹股沟疝住院患者,按随机数字表法分为观察组和对照组,每组40例。对照组患者采用疝囊高位结扎疝修补术,观察组患者采用腹膜外腹腔镜疝气修补术,观察比较两组患者手术时间、住院天数、置管时间、术中出血量和住院总费用,术后1 d和3 d按视觉模拟评分法(VAS)评分,并发症发生率及1年内复发率。结果 观察组患者在手术时间、住院天数、置管时间、术中出血量和住院总费用均显著低于对照组。观察组术后1 d和3 d VAS评分显著低于对照组(P<0.05),观察组患者并发症发生率(10.0%)显著低于对照组患者(32.5%),P<0.05。术后随访调查显示,两组患者1年复发率差异无统计学意义(P>0.05)。结论 腹膜外腹腔镜疝气修补术治疗老年腹股沟疝疗效显著,具有创伤小,恢复快,费用低,并发症少和复发率低等优点。 |

| Objective To explore the application value of laparoscopic extraperitoneal herniorrhaphy in elderly with inguinal hernia.Methods Eighty cases of elderly inpatients with unilateral inguinal hernia were selected and randomly divided into observation group and control group,with 40 cases in each group,according to random number table method.Control group was treated with high ligation of hernia sac and hernia repair,and observation group was treated with laparoscopic extraperitoneal herniorrhaphy.The operative parameters (operative time,hospital stay,catheterization time,amount of intraoperative blood loss and total hospitalization cost),VAS scores at 1 d and 3 d after operation,incidence rate of complications and recurrence rate within 1 year were observed and compared between the two groups.Results The operative time,hospital stay,catheterization time,amount of intraoperative blood loss and total hospitalization cost in the observation group were significantly lower than those in the control group.The VAS scores at 1 d and 3 d after operation in the observation group were significantly lower than those in the control group (P<0.05),and the incidence rate of complications in the observation group was significantly lower than that in the control group (10.0% vs 32.5%) (P<0.05).Postoperative follow-up revealed that there was no statistically difference in the 1-year recurrence rate between the two groups (P>0.05).Conclusion Laparoscopic extraperitoneal herniorrhaphy has significant efficacy with small trauma,quick recovery,low cost,few complications and low recurrence rate in the treatment of the elderly with inguinal hernia. |
