| 目的 探讨远程家庭血压监测及干预在控制社区高血压患者中的应用效果。方法 在社区高血压人群中选择服用降压药物但血压控制效果不良的患者30例,按照随机数字表法分为干预组(15例)和对照组(15例)。两组高血压患者均采用远程家庭血压监测,在此基础上对照组采用传统社区高血压管理,干预组则实施安徽医科大学大学第一附属医院下沉基层的高年资护士配合社区医生管理社区高血压患者,干预1、3个月后比较研究前后两组患者血压控制的水平。结果 实验1个月两组收缩压(SBP)、舒张压(DBP)差异无统计学意义(P>0.05)；实验3个月后干预组SBP较对照组SBP有明显改善,差异有统计学意义(P＜0.05),DBP较对照组DBP差异无统计学意义；实验1、3个月患者SBP、DBP均较实验前有较明显的改善,两两比较差异有统计学意义(P＜0.05)。结论 远程家庭血压监测可以有效控制高血压患者的血压水平,在此基础上施行高年资护士配合社区医生管理社区高血压患者能更大程度改善患者血压水平。 |

| Objective To explore the effect of home blood pressure telemonitoring and intervention in controlling hypertension in community.Methods 30 patients with hypertension in the community who took hypertensive drugs but had poor blood pressure control randomly divided into intervention group (15 cases) and control group (15 cases),both of the two groups of patients with high blood pressure adopt home blood pressure telemonitoring,then the control group adopt traditional community hypertension management while the intervention group,on this basis,joined the intervention of nurse practitioner(NP).The blood pressure control levels of the two groups were compared after 1 and 3 months of intervention.Results There was no significant difference in SBP and DBP between the two groups within 1 month (P>0.05),and 3 months after the trial,the SBP of the intervention group was significantly improved compared with that of the control group,and the difference was statistically significant(P<0.05).The SBP and DBP of patients were significantly improved after 1 and 3 months compared with those before the experiment,and the difference was statistically significant(P<0.05).Conclusions Home blood pressure telemonitoring can effectively help control the blood pressure of hypertensive patients.In addition to the management of hypertension patients by community doctors,the cooperation of NP can improve the blood pressure of patients to a greater extent. |
