| 目的 探讨慢性心力衰竭血清胱抑素C(CysC)和B型利钠肽(BNP)水平变化及相关性,评估两者在心力衰竭患者心功能下降严重程度及预后的临床意义。方法 选取新疆医科大学第一附属医院心脏中心2017年1月至12月收治的339例慢性心力衰竭患者作为心力衰竭组。心力衰竭组根据左心室射血分数(LVEF)分为3个亚组:舒张功能不全组(HFpEF)、中间射血分数组(HFmrEF)及射血分数降低组(HFrEF)。选取126例健康体检人员作为对照组。采用酶联免疫吸附法检测研究对象血清CysC水平,化学发光微粒子免疫检测法测定BNP水平。结果 与对照组比较,心力衰竭各亚组血清CysC、BNP水平均高于对照组,差异有统计学意义(P<0.05)。HFmrEF和HFrEF组左室舒张末期内径(LVEDd)明显高于对照组,差异有统计学意义(P<0.05)。HFpEF组LVEDd与对照组差异无统计学意义(P>0.05)。与HFpEF组比较,心力衰竭亚组HFmrEF和HFrEF组血清CysC、BNP水平均高于HFpEF组,差异有统计学意义(P<0.05)。HFmrEF和HFrEF组LVEDd明显高于HFpEF组,差异有统计学意义(P<0.05)。Pearson相关性分析显示,CHF患者CysC与BNP呈正相关。结论 慢性心力衰竭随着左心室射血分数的降低血清CysC、BNP进行性升高,且两者间呈正相关。血清CysC、BNP联合检测可作为评估慢性心力衰竭患者病情严重程度及预后的重要指标。 |

| Objective To investigate the changes and correlation of serum cystatin C and BNP levels in patients with chronic heart failure and evaluate their clinical significance in the severity of heart failure and prognosis.Methods 339 patients with chronic heart failure admitted to the heart center of the First Affiliated Hospital of Xinjiang Medical University from January to December 2017 were selected as the heart failure group.According to left ventricular ejection fraction (LVEF),the heart failure group was divided into three subgroups:diastolic dysfunction group (HFPEF group),intermediate ejection fraction group (HEmrEF group) and ejection fraction reduction group (HErEF group).126 healthy persons (control group).Enzyme linked immunosorbent assay (ELISA) and chemiluminescent particle immunoassay (chemiluminescent particle immunoassay) were used to detect the levels of CysC and BNP.Results Compared with the control group,the levels of serum CysC and BNP in each subgroup of heart failure were higher than those in the control group,and the difference was statistically significant (P<0.05).The LVEDd of HFmrEF and HErEF group was significantly higher than that of the control group (P<0.05).There was no significant difference in LVEDd between HFpEF group and control group (P>0.05).Compared with HFpEF group,the levels of serum CysC and BNP in HEmrEF and HErEF group were significantly higher than those in HFpEF group (P<0.05).LVEDd of HEmrEF and HErEF group was significantly higher than that of HFpEF group (P<0.05).Pearson correlation analysis showed that there was a positive correlation between CysC and BNP in CHF patients.Conclusion With the decrease of left ventricular ejection fraction,serum CysC and BNP were increased in chronic heart failure.The combined detection of serum CysC and BNP can help evaluate the severity and prognosis of CHF. |
