| 目的 探讨全髋关节置换术(THA)和半髋关节置换术(HA)治疗老年股骨颈骨折的效果。方法 选取2018年1月至2019年1月大连市第二人民医院收治的104例老年股骨颈骨折患者。按随机数字表法分为对照组与研究组,各52例。对照组采用HA治疗,研究组采用THA治疗。比较两组治疗效果。结果 对照组手术时间短于研究组,术中出血量少于研究组(P<0.05)；对照组术后并发症发生率19.23%,高于研究组3.85%(P<0.05)；术后3 d,对照组髋关节疼痛程度高于研究组(P<0.05)；术后6个月、12个月,对照组髋臼前倾角小于研究组,髋臼外展角大于研究组,Harris髋关节功能评分低于研究组(P<0.05)。结论 THA具有疼痛程度轻、并发症少、关节功能较好等特点,但手术时间长、出血多；而HA具有手术时间短、出血少,但术后疼痛明显,并发症多,关节功能恢复慢。 |

| Objective To investigate the effect of total hip arthroplasty (THA) and hemiarthroplasty (HA) in the treatment of femoral neck fracture in the elderly.Methods 104 elderly patients with femoral neck fracture were selected from January 2018 to January 2019 and randomly divided into control group and study group,52 cases each.HA was used in the control group and THA in the study group.The therapeutic effects of the two groups were compared.Results The operation time of the control group was shorter than that of the study group,and the amount of intraoperative hemorrhage was less than that of the study group (P<0.05); the incidence of postoperative complications of the control group was 19.23%,higher than that of the study group (P<0.05); the degree of hip joint pain of the control group was higher than that of the study group (P<0.05) 3 days after operation; 6 and 12 months after operation,the acetabulum anteversion angle of the control group was smaller than that of the study group,the acetabulum abduction angle was larger than that of the study group, Harris hip function score was lower than the study group (P<0.05).Conclusion THA has the characteristics of light pain,less complications and better joint function,but the operation time is longer and the bleeding is more; HA has the characteristics of shorter operation time and less bleeding,but the postoperative pain is obvious with more the complications more shower and the joint function recovery is slow. |
