目的 探讨脑梗死患者血栓弹力图(TEG)各项参数与凝血试验指标的相关性。方法 纳入江油市中医医院2017年10月至2019年10月收治的118例脑梗死患者作为研究组,并以同期门诊体检的118例健康人员作为对照组,对比两组TEG检测结果、凝血试验检测结果。Spearman分析TEG、凝血试验相关性。结果 研究组凝血综合指数(CI,1.72±0.41)、血凝块最大强度或硬度[MA,(69.62±5.44)mm]、血细胞凝集块形成速率[Angle,(68.26±3.62)deg]、纤维蛋白原[FIB,(3.62±0.36)g/L]均显著比对照组高,P<0.05(t=72.790、14.348、23.359、27.511,P值均<0.05)。研究组血细胞凝集块形成时间[K值,(1.15±0.12)min]、凝血反应时间[R值,(5.32±0.17)min]、活化部分凝血活酶时间[APTT,(28.12±1.62)s]、凝血酶原时间国际标准化比值(PT-INR,0.65±0.11)、凝血酶原时间[PT,(11.36±1.11)s]均显著比对照组[K值(2.28±0.31)min、R值(6.89±0.44)min、APTT(30.26±2.22)s、PT-INR(0.96±0.16)、PT(12.68±2.01)s]低,P<0.05(t=36.927、18.889、8.459、17.343、6.245,P值均<0.05)。TEG K值、R值与PT-INR为正相关性(P<0.05),与纤维蛋白为负相关性(P<0.05)。结论 脑梗死患者进行TEG、凝血试验检测,两种检测方法均具有一定的优势,可弥补各自的不足。 |

Objective To analyze the correlation between thromboelastography (TEG) parameters and coagulation test indexes in patients with cerebral infarction.Methods 118 patients with cerebral infarction admitted to our hospital from October 2017 to October 2019 were selected as the experimental group,and 118 healthy people from outpatient physical examination in the same period were taken as the control group.The TEG test results and coagulation test results of the two groups were compared.The correlation between the TEG test results and coagulation test results are analyzed by Spearman rank correlation analysis method.Results The coagulative index(CI) (1.72±0.41),MA (69.62±5.44) mm,Angle (68.26±3.62) deg and FIB (3.62±0.36) g/L in the experimental group were significantly higher than those in the control group CI [(0.91±0.14),MA (61.25±3.25) mm,angle (59.25±2.11) deg,FIB (2.51±0.25) g/L] with P<0.05 (t=72.790,14.348,23.359,27.511,all P<0.05).K value (1.15±0.12) min,R value (5.32±0.17) min,APTT (28.12±1.62) s,PT-INR (0.65±0.11),Pt (11.36±1.11) s in the experimental group were significantly lower than those in the control group [K value (2.28±0.31) min,R value (6.89±0.44) min,APTT (30.26±2.22),PT-INR (0.96±0.16),Pt (12.68±2.01) ] with P<0.05 (t=36.927,18.889,8.459,17.343,6.245,all P<0.05).TEG K value and R value were positively correlated with PT-INR (P<0.05),and negatively correlated with fibrin (P<0.05).Conclusion TEG and coagulation test in patients with cerebral infarction have certain advantages and can make up for their shortcomings. |
