      目的 探究修正的Geneva评分、Daniel心电图评分与D-二聚体(D-D)预测急性肺栓塞的临床意义。方法 选取85例疑似急性肺栓塞患者作为研究对象,经CT肺动脉造影,确诊49例。借助受试者工作特征(ROC)曲线,依次对修正的Geneva评分、Daniel心电图评分、D-D能否预测肺栓塞实施评价。结果 修正的Geneva评分越高,肺栓塞可能性越大；Daniel心电图评分以2分为界点,小于则提示肺栓塞可能性小,反之可能性增加；修正的Geneva评分、Daniel心电图评分结合D-D对肺栓塞的阴性预测值分别为22.22%和38.89%。结论 在肺栓塞中,修正的Geneva评分、Daniel心电图评分、D-D预测价值高,综合修正的Geneva评分和D-D,能够排除肺栓塞可能。
英文摘要:
      Objective To explore the clinical significance of modified Geneva score,Daniel electrocardiogram score and D-Dimer (D-D) in predicting pulmonary embolism.Methods 85 patients with suspected pulmonary embolism were selected as the research objects,and 49 cases were confirmed as pulmonary embolism by CT pulmonary angiography.With the help of receiver operating characteristic (ROC) curve,we evaluated whether the modified Geneva score,Daniel ECG score and D-D could predict pulmonary embolism.Results The higher the modified Geneva score,the greater the possibility of pulmonary embolism.Daniel ECG score with 2 points as the cut-off point,less than the cut-off point indicated that the possibility of pulmonary embolism was small,on the contrary,the possibility increased.Meanwhile,the negative predictive values of modified Geneva score,Daniel ECG score combined with D-D for pulmonary embolism were 22.22% and 38.89%,respectively.Conclusion The modified Geneva score,Daniel ECG score and D-D have high value in predicting pulmonary embolism.The comprehensive modified Geneva score and D-D is helpful to exclude pulmonary embolism.
