| 目的 探讨住院老人衰弱与生活质量之间的关系。方法 根据Edmonton衰弱评估量表评分将患者分为无衰弱组、轻度衰弱组、中度衰弱组、重度衰弱组,应用 SF-36量表对老年住院患者生活质量进行评估,分析衰弱状况对生活质量的影响。结果 64例住院患者中11例(17.2%) 无衰弱,16例(25%)为轻度衰弱,19例 (29.7%) 为中度衰弱,18例(28.1%)为重度衰弱。衰弱程度越严重的患者,年龄越大,用药种类越多(P<0.05)。重度衰弱组与无衰弱组、轻度衰弱组、中度衰弱组比较,除一般健康状况(GH)维度外其他生活质量维度间差异均有统计学意义(P<0.05),其中活力下降最为显著。重度衰弱患者在躯体功能、躯体职能、活力、社会职能、情感职能、精神健康共6方面的评分明显低于其余三组,差异均有统计学意义(P<0.05)。无衰弱的患者其健康变化较其他三组差异有统计学意义(P＜0.05)。患有衰弱的住院患者生活质量各维度均低于无衰弱综合征患者(P＜0.05)。结论 衰弱对老年患者的生活质量具有很大影响,在照护住院老年患者时,临床医务人员应主动了解患者的衰弱情况,全面评估,提供有针对性、个体化的照护措施,以有效地提高患者的生活质量。 |

| Objective To explore the relationship between frailty and quality of life in elderly in-hospital patients.Methods Patients were divided into four groups by Edmonton frailty scale:no frailty group,mild frailty group,moderate frailty group and severe frailty group.SF-36 scale was used to evaluate the quality of life and analyze the impact of frailty on their quality of life.Results Among the 64 inpatients,11 (17.2%) had no frailty,16 (25%) had mild frailty,19 (29.7%) had moderate frailty,and 18 (28.1%) had severe frailty.The higher degree of frailty presented with older age and more kinds of drugs used (P<0.05).In addition to general health (GH),the scores of other dimensions of SF-36 decreased significantly with the severity of the frailty among the four groups,in which VT decreased mostly (P<0.05).All the scores of different dimensions of the quality of life in frailty elderly patients were lower than the patients without frailty (P<0.05).Conclusion Frailty has a great influence on the quality of life of the elderly patients.When caring for the elderly patients in hospital,the clinical medical staff should try to detect frailty in the patients,carry the geriatric comprehensive assessment,and provide targeted and individualized interventions to effectively improve the quality of life of frailty the patients. |
