| 目的 分析老年肿瘤患者自杀死亡率的特征及趋势,为有效预防提供依据。方法 从美国SEER(Surveillance,Epidemiology,and End Result)数据库中提取1975—2016年诊断为肿瘤的患者,分为≥60岁组和＜60岁组。以同期美国总体人群自杀死亡率为参考计算标准死亡比(SMR),回顾性分析老年肿瘤患者自杀死亡率的特征,与人口统计学资料、肿瘤诊断等因素的关系。结果 1975—2016年,美国总体人群自杀死亡率为1.20/万。提取SEER数据库同期肿瘤患者8 077 061人,因自杀死亡患者11 615人,自杀死亡率为14.38/万,SMR=11.98。≥60岁组和＜60岁组,自杀死亡率分别为14.56/万(SMR=8.56)、14.03/万(SMR=12.75)。近40年来,肿瘤患者自杀死亡率呈明显下降趋势。男性、年龄70岁左右、患有高症状负担肿瘤的患者自杀死亡率较高。结论 老年肿瘤患者中,男性、高症状负担肿瘤类型及70岁左右患者是重点关注人群,需引起重视,警惕自杀倾向。 |

| Objective To analyze the characteristics and trend of suicide rate in elderly cancer patients,so as to provide evidence for effective prevention.Methods Patients diagnosed with cancer from 1975 to 2016 were extracted from SEER database and divided into≥60 years old group and＜60 years old group.The standard mortality rate (SMR) was calculated based on the suicide rate of the general population in the United States.The characteristics of suicide rate in elderly cancer patients and its relationship with demographic data,tumor diagnosis and other factors were analyzed retrospectively.Results From 1975 to 2016,the suicide rate was 12.00 per 10 000 in US.The SEER database was extracted from 8 077 061 cancer patients in the same period,of which 11 615 patients were died of suicide.The suicide rate was 14.38 per 10 000,and the SMR was 11.98.Among them,the suicide rate was 14.56 per 10 000(SMR=8.56) in≥60 years old cancer patients group,and 14.03 per 10 000(SMR=12.75) in＜60 years old cancer patients group.The suicide rate of cancer patients who are about 70 years old,male,head and neck cancer with high symptom burden was higher.Conclusion The suicide rate of elderly cancer patients is 11.98 times higher than that of the general population.Among the elderly cancer patients,male,high symptom burden tumor type and patients about 70 years old are the focus people,which should be paid attention to and vigilant against suicidal tendency. |
