目的 分析对急性呼吸窘迫综合征(ARDS)患者呼出气一氧化氮(NO)水平检测的临床意义。方法 以2018年1月至12月在山东省第二人民医院西院急诊科就诊的ARDS患者59例为观察组,以同时期来该院健康体检的志愿者22例为对照组;收集两组的急性生理及慢性健康状况评分Ⅱ(APACHE Ⅱ)、氧合指数(PaO2/FiO2)、呼出气NO水平等指标并比较;观察组患者再根据收治时ARDS病情严重程度及治疗后存活情况分别进行分组,比较不同亚组间呼出气NO水平差异。结果 治疗前观察组呼出气NO水平显著高于对照组(P<0.05);观察组治疗后呼出气NO水平显著低于治疗前(P<0.05);观察组按照ARDS病情严重程度分为轻度组26例,中度组20例,重度组13例,结果显示患者病情越严重,呼出气NO水平越高,依次为重度组>中度组>轻度组;在治疗1周后根据存活情况分为死亡组14例,存活组45例,死亡组患者末次呼出气NO检测水平显著高于存活组患者(P<0.05);相关性分析显示,ARDS患者呼出气NO水平与APACHE Ⅱ评分呈正相关(r=0.417,P<0.05),与PaO2/FiO2呈负相关性(r=-0.682,P<0.05)。结论 呼出气NO水平能够反映ARDS病情严重程度,对预后判断有一定的指导作用。 |

Objective To analyze the clinical significance of determination of nitric oxide (NO) in exhaled breath in patients with acute respiratory distress syndrome (ARDS).Methods Fifty-nine patients with ARDS admitted to the emergency department of Shandong Second Provincial General Hospital from January to December 2018 were selected as the observation group,and 22 volunteers who came to our hospital for physical examination during the same period were selected as the control group.APACHE Ⅱ,PaO2/FiO2,and exhaled NO of two groups were collected and compared.The difference of NO level between different subgroups was compared to evaluate the application value of NO detection in ARDS.Results The exhaled NO level of the observation group was significantly higher than that of the control group before treatment (P<0.05);the exhaled NO level of the observation group was significantly lower than before treatment after treatment (P<0.05).According to the severity of ARDS,the observation group was divided into a mild group of 26 cases,a moderate group of 20 cases,and a severe group of 13 cases.The results showed that the more severe the illness was,the higher the level of NO in the exhumed breath was,followed by the severe group > moderate group > mild group.After 1 week of treatment,14 patients were divided into the death group and 45 patients into the survival group according to survival.The NO detection level of last exhale in the death group was significantly higher than that in the survival group (P<0.05).Correlation analysis showed that the exhaled NO level of ARDS patients was positively correlated with the APACHE Ⅱ score (r=0.417,P<0.05),had a negative correlation with PaO2/FiO2 (r=-0.682,P<0.05).Conclusion The detection of exhaled NO level is significance for the diagnosis of ARDS,It can reflect the severity of ARDS and help evaluate the prognosis of ARDS. |
