      目的 研究老年科医院获得性肺炎患者(HAP)的病原学分布与耐药情况,为老年HAP预防及临床抗感染策略制订提供依据。方法 回顾性分析广州市第一人民医院老年科自2018年8月到2019年8月确诊HAP患者189例气道分泌物的病原体培养及药物敏感试验结果。结果 广州市第一人民医院老年科HAP致病菌以革兰阴性杆菌为主(占92.72%),铜绿假单胞菌为最常见病原菌,前五位致病菌依次为:铜绿假单胞菌(28.48%)、流感嗜血杆菌(13.25%)、鲍曼不动杆菌(10.6%)、大肠埃希氏菌(10.6%)、肺炎克雷伯菌(9.27%);并且除流感嗜血杆菌外,常见病原菌的多重耐药菌比例高;合并不同基础疾病的老年HAP患者主要病原菌及耐药情况不同。结论 老龄HAP患者的病原菌以革兰阴性菌为主,多重耐药比例高,应结合基础疾病制定合理的防治HAP策略。

      Objective To study the pathogenic microorganism distribution and drug resistance of patients with hospital-acquired pneumonia (HAP) in geriatric department,and to provide a basis for the prevention and clinical strategies of HAP in the elderly.Methods The pathogen culture and drug sensitivity test results of 189 airway retrospectively analysed in HAP patients from August 2018 to August 2019 in Geriatrics Department of Guangzhou First People's Hospital.Results The gram-negative bacilli (92.72%) was the main pathogens of HAP in the geriatric department.Pseudomonas aeruginosa was the most common pathogen,and the top five pathogens were pseudomonas aeruginosa (28.48%),haemophilus influenzae (13.25%),acinetobacter baumannii (10.6%),escherichia coli (10.6%) and klebsiella pneumoniae (9.27%).Except for haemophilus influenzae,the proportion of multi-drug resistance in the common pathogens was high.Besides,the main pathogens and drug resistance in elderly HAP patients with various combined diseases were different.Conclusions Gram negative bacteria are the main pathogens in elderly patients with HAP,and the proportion of multi drug resistance of pathogens is high.It is necessary to formulate reasonable prevention and control strategies for HAP based on the underlying diseases.
