目的 探讨老年男性2型糖尿病患者维生素D营养状况及2型糖尿病患者合并骨质疏松的危险因素。方法 选取2015年2月至2018年6月来上海交通大学医学院附属新华医院就诊的516例老年男性2型糖尿病患者,根据是否伴有骨质疏松分为糖尿病骨质疏松组(n=151)和非骨质疏松组(n=365),分析所有人群维生素D营养状况;比较两组年龄、病程、体质指数(BMI)、25羟维生素D[25(OH)D]、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FIns)、血钙(Ca)、血磷(P)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、总胆固醇(TC)、三酰甘油(TG)等指标;并分析2型糖尿病合并骨质疏松的危险因素。结果 在516例老年男性2型糖尿病患者中,维生素D缺乏者占68.41%(包括严重缺乏);不足者占24.81%;充足者占6.78%。糖尿病骨质疏松组年龄、病程、HbA1c、TC、TG高于非骨质疏松组,25(OH)D、FIns低于非骨质疏松组,差异均有统计学意义(P<0.05)。logistic回归分析显示年龄增长、病程增加、HbA1c升高及维生素 D缺乏是2型糖尿病合并骨质疏松的危险因素(P<0.05)。结论 老年男性2型糖尿病患者普遍存在维生素D缺乏;随年龄增长、病程增加和HbA1c升高,维生素D缺乏的2型糖尿病患者更容易发生骨质疏松。 |

Objective To explore the nutritional status of vitamin D in elderly male patients with type 2 diabetes and the risk factors of osteoporosis in patients with type 2 diabetes.Methods A total of 516 elderly male patients with type 2 diabetes admitted to Shanghai Jiao Tong University School of Medicine from February 2015 to June 2018 were selected and divided into diabetic osteoporosis group(n=151) and non-osteoporotic group(n=365) according to the osteoporosis.The vitamin D nutritional status of all groups was analyzed.Age,course of disease,body mass index(BMI),25(OH)D,fasting blood glucose(FPG),hemoglobin a1c(HbA1c),fasting insulin(FIns),blood calcium (Ca),blood phosphorus(P),low-density lipoprotein(LDL),high-density lipoprotein(HDL),total cholesterol(TC),triglyceride(TG) and other indexes were compared between the two groups.The risk factors of type 2 diabetes mellitus with osteoporosis were analyzed.Results Among 516 elderly male patients with type 2 diabetes,Vitamin D deficiency accounted for 68.41% (including severe deficiency);24.81% were deficient;adequate accounted for 6.78%.The age,course of disease,HbA1c,TC and TG in diabetic osteoporosis group were higher than those in non-osteoporosis group,and 25(OH)D and FIns were lower than those in non-osteoporosis group,the differences were statistically significant (P<0.05).Logistic regression analysis showed that age,increasing course of disease,increased HbA1c and vitamin D deficiency were risk factors for osteoporosis in type 2 diabetes mellitus (P<0.05).Conclusions Vitamin D deficiency is common in elderly men with type 2 diabetes.Type 2 diabetes mellitus patients are more likely to develop osteoporosis with increasing age,disease duration,elevated HbA1c and vitamin D deficiency. |
