目的 探讨趋化因子CCL2对老年ST段抬高型心肌梗死(STEMI)患者服用替格瑞洛后残余血小板活化的影响。方法 选取北部战区总医院收治的老年STEMI患者80例,服用180 mg替格瑞洛,采集患者临床资料,抽取患者服药后2、4、6 h静脉血。采用Verifynow法检测P2Y12反应单位(PRU)。根据数值不同,将患者分为残余血小板高反应组(高反应组)和残余血小板正常反应组(正常反应组)。采用ELISA法检测患者血浆CCL2浓度。患者血浆CCL2浓度与PRU值之间的关系采用Pearson相关分析。结果 两组患者临床基线各指标之间差异无统计学意义(P>0.05)。服用替格瑞洛后2、4、6 h,患者残余血小板高反应比例分别为38.8%、18.8%、17.7%。服药后不同时间点高反应组患者血浆中CCL2浓度[2 h时为(212.10±88.81)pg/mL;4 h时为(210.00±73.00)pg/mL;6 h时为(200.00±48.57)pg/mL]均明显高于正常反应组患者[2 h时为(169.50±36.59)pg/mL;4 h时为(161.60±40.76)pg/mL;6 h时为(152.10±44.05)pg/mL,P<0.01]。服药后2、4、6 h两组患者血浆CCL2浓度与PRU之间均存在线性关系,r=0.463,r=0.622,r=0.413,P<0.01)。结论 趋化因子CCL2参与了老年STEMI患者残余血小板的聚集和活化。 |

Objective To investigate the influence of chemokine CCL2 on residual platelet activation in the elderly patients with ST-elevation myocardial infarction (STEMI).Methods The baseline clinical data of 80 elderly STEMI patients taking 180 mg ticagrelor were collected in the General Hospital of North Theater.The P2Y12 reaction unit (PRU) of the patients was detected at different time points (2 h,4 h,6 h) after taking the medicine using Verifynow method,and the patients were divided into two groups according to the results.The plasma CCL2 concentration in the high reactivity group and normal reactivity group was detected by ELISA.The correlation of CCL2 concentration in plasma and PRU at different time points was analyzed by Pearson correlation.Results There was no significant difference of each data between normal reactivity group and high reactivity group (P>0.05).The proportion of high residual platelet reactivity of the patients was 38.8%,18.8% and 17.7% respectively at different time points (2 h,4 h,6 h) after taking ticagrelor.The plasma CCL2 concentration in the high reactivity group [2 h:(212.10±88.81) pg/mL,4 h:(210.00±73.00)pg/mL,6 h:(200.00±48.57)pg/mL] was higher than that in the normal reactivity group [2 h:(169.50±36.59) pg/mL,4 h:(161.60±40.76) pg/mL,6 h:(152.10±44.05) pg/mL] at different time points after taking ticagrelor (P<0.01).There was a linear correlation between the CCL2 concentration in plasma and PRU (r=0.463,r=0.622,r=0.413,P<0.01).Conclusion Chemokine CCL2 participates in the aggregation and activation of residual platelets in the elderly patients of STEMI. |
