| 目的 分析无症状结直肠息肉患者的血糖、血脂、血尿酸水平的变化,观察息肉对机体代谢的影响。方法 回顾性分析2014年6月至2020年10月于解放军第三○五医院行纤维结肠镜的355例无症状体检者病历资料,其中行内镜下结直肠息肉切除术160例。相关指标分析包括空腹血糖(FBG)、三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、血尿酸(SUA)。分别比较息肉组与健康对照组、不同数量息肉组、不同部位息肉组、不同大小息肉组及不同病理类型息肉组间各指标水平的情况。结果 息肉组的TG、SUA指标水平皆高于对照组(t=2.387,P＜0.05；t=2.101,P＜0.05),息肉组的HDL-C指标水平低于对照组(t=2.221,P＜0.05)；近端结肠息肉组的LDL-C指标水平高于远端结肠+直肠息肉组(t=2.071,P＜0.05)；大息肉组的TC、LDL-C指标水平皆高于小息肉组(t=2.059,P＜0.05；t=2.518,P＜0.05)；非腺瘤性息肉组的TG、SUA指标水平皆高于腺瘤性息肉组(t=2.001,P＜0.05；t=2.341,P＜0.05)；各组间血糖水平差异无统计学意义。结论 结直肠息肉患者的血脂、血尿酸水平升高,近端结肠息肉、大息肉、非腺瘤性息肉的患者变化更为明显。 |

| Objective The changes of blood glucose,lipid and uric acid levels in asymptomatic colorectal polyps were analyzed to observe the effects of polyps on metabolism.Methods The data of 355 asymptomatic patients who underwent fibrocolonoscopy in the 305th Hospital of the Chinese People′s Liberation Army from June 2014 to October 2020 were retrospectively analyzed.Among them,160 patients underwent endoscopic colorectal polypectomy.According to the numbers,location,sizes and pathology,all the patients were divided into the different groups.Analyze the indexes of the colorectal polyps-patinets indifferent groups.The indexes included the fasting blood glucose (FBG),triglyceride (TG),total cholesterol (TC),high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol (LDL-C) and serum uric acid (SUA).Results The levels of TG and SUA in polyps group were higher than those in the control group (t=2.387,P＜0.05;t=2.101,P＜0.05),and the levels of HDL-C in polyp group were lower than those in the control group (t=2.221,P＜0.05).The levels of LDL-C in the proximal colonic polyps group were higher than those in the distal colorecta1 polyps group (t=2.071,P＜0.05).The TC and LDL-C in the large polyps group were higher than those in the small polyps group (t=2.059,P＜0.05;t=2.518,P＜0.05).The TG and SUA in the non-adenomatous polyps group were higher than those in the adenomatous polyps group (t=2.001,P＜0.05;t=2.341,P＜0.05).Blood glucose had no significant difference among groups(P>0.05).Conclusions The levels of blood lipids and serum uric acid is increased in patients with colorectal polyps,but the blood glucose has no significant change.Among them,patients with proximal colonic polyps,large polyps and non-adenomatous polyps have more obvious changes in each index. |
