| 老年综合评估是综合医院评估虚弱老年人的“金标准”,在老年人的连续医疗中得到了广泛应用,但在社区开展老年综合评估尚未有统一标准。本文介绍了老年综合评估在社区应用的特点、几种适用于社区环境的评估工具,以及各种评估工具在社区最常见的老年综合征中的应用。在基层健康管理中心,由全科医生为主导,调动社区有利资源,运用适合社区的老年综合评估对老年人进行长期照顾,能够有益于老年人社区管理,有益于老年人的身心健康。 |

| At present,the comprehensive geriatric assessment(CGA) is the “gold standard” for general hospitals to evaluate the frail elderly,which has been widely used in the continuous care of the elderly.However,there is no unified standard to carry out CGA in the community.This paper introduces the characteristics of CGA in community,several assessment tools suitable for community environment,and the application of various assessment tools in the most common geriatric syndromes in community.In the primary health management center,it is beneficial to the community management and the physical and mental health of the elderly by mobilizing the favorable resources of the community and using the comprehensive assessment of the elderly suitable for the community to provide long-term care for the elderly,led by general practitioners. |
