| 目的 探讨复方真武汤联合常规西药治疗慢性心力衰竭的效果。方法 选取2019年1月至2020年12月上海中医药大学附属市中医医院和上海市浦东新区人民医院收治的86例老年慢性心力衰竭患者为研究对象。按照随机数字表法分为对照组(43例)和观察组(43例)。对照组予以贝沙坦等常规西药治疗,观察组予以厄贝沙坦等常规西药+复方真武汤治疗,比较两组临床疗效、明尼苏达心力衰竭生活质量问卷评分(MLHFQ)、炎症细胞因子[白细胞介素6(IL-6)、血清淀粉样蛋白A(SAA)]、同型半胱氨酸(Hcy)、心功能指标[脑钠肽(BNP)、左室射血分数(LVEF)、心排出量(CO)、心脏指数(CI)]、6 min步行实验(6MWT)。结果 治疗后,观察组的临床总有效率、MLHFQ评分高于对照组,差异有统计学意义(P＜0.05)；血清中IL-6、Hcy、SAA、BNP等检测水平均低于对照组患者；心功能指标(LVEF、CO、CI、6MWT)均有优于对照组,差异有统计学意义(P＜0.01)。两组均无明显不良反应(P＞0.05)。结论 复方真武汤联合厄贝沙坦等药物能显著改善老年慢性心力衰竭患者临床症状和生活质量,降低血清中炎症因子,并能显著改善心功能。 |

| Objective To explore the clinical effect of Zhenwu Decoction Combined with conventional western medicine on senile chronic heart failure (HFMREF) with median ejection fraction.Methods In this study,a total of 86 elderly patients with chronic heart failure admitted to the Hospital of Traditional Chinese Medicine Affiliated to Shanghai University of Traditional Chinese Medicine and the Emergency Center of Shanghai Pudong New Area People′s Hospital from January 2019 to December 2020 were selected as the research subjects.According to the random number table method,they were divided into control group (n=43) and observation group (n=43).The control group was treated with conventional western medicine,and the observation group was treated with conventional western medicine + compound Zhenwu Decoction.Clinical efficacy,Minnesota Heart Failure Quality of Life Questionnaire (MLHFQ),and inflammatory cytokines were compared between the two groups:[IL-6,serum amyloid A protein (SAA)],Hcy,cardiac function indicators:[Brain natriuretic peptide (BNP),left ventricular ejection fraction (LVEF),cardiac output (CO) and cardiac index (CI)],6-minutes Walk Test (6MWT).Results After treatment,the clinical total effective rate and mLHFQ score of the observation group were higher than those of the control group with statistical significance (P<0.05).The levels of IL-6,Hcy,SAA and BNP in serum were lower than those in control group.Cardiac function indexes (LVEF,CO,CI,6MWT) were better than those in the control group with statistical significance (P<0.01).Conclusions Compound Zhenwu Decoction combined with irbesartan and other conventional western drugs can significantly improve the clinical symptoms and quality of life among elderly patients with chronic heart failure,reduce the inflammatory factors in serum,and significantly improve cardiac function. |
