目的 探讨脑小血管病导致的认知功能障碍患者血清胱抑素C(CysC)及炎症因子的改变及意义。方法 选取2019年3月至2020年1月于抚顺市中心医院神经内科住院65岁以上的老年脑小血管病患者76例以及健康体检者30例(对照组),对脑小血管病患者用蒙特利尔认知功能表(MoCA) 进行认知功能评定,根据MoCA评分情况将患者分为两组,认知功能障碍组(VCI)37例,无认知功能障碍组(non-VCI)39例,并对三组患者的CysC及血清炎症因子白细胞介素1β(IL-1β)、肿瘤坏死因子α(TNF-α)进行对比分析,计算其与认知功能障碍的相关性。结果 VCI组患者血清CysC、IL-1β以及TNF-α水平显著高于non-VCI组和对照组(P<0.05)。经相关性分析后发现,血清CysC、IL-1β以及TNF-α水平均与MoCA量表总分呈负相关。结论 血清CysC、IL-1β以及TNF-α水平与脑小血管病的认知功能相关,通过测定脑小血管病患者这三种物质水平对认知功能障碍的判定起到一定的辅助作用。 |

Objective To investigate the changes and significance of serum cystatin C and inflammatory factors in patients with cognitive impairment caused by cerebrovascular disease.Methods Seventy six cases of aged patients over 65 years old with cerebrovascular disease and 30 cases of healthy patients (control group) who were hospitalized in the department of neurology of Fushun Central Hospital from March 2019 to January 2020 were selected,the cognitive function of patients with cerebrovascular disease was assessed by using the Montreal cognitive function sheet (MoCA),the patients were divided into two groups:37 patients in the cognitive impairment group (VCI) and 39 patients in the non-vci group according to the MoCA score,the correlation between CysC,interleukin-1β (IL-1β) and Tumor Necrosis factor-α (TNF-α) in the three groups was compared and analyzed to calculate the correlation between CysC,interleukin-1β (IL-1β) and Tumor Necrosis factor-α (TNF-α).Results Serum levels of CysC,IL-1β and TNF-α in the VCI group were significantly higher than those in the non-VCI group and the control group (P<0.05).Correlation analysis,showed that serum levels of CysC,IL-1β and TNF-α were negatively correlated with the total score of MoCA scale.Conclusion Serum levels of CysC,IL-1β and TNF-α are related to the cognitive function of patients with cerebrovascular disease,and the determination of these three substances can play an auxiliary role in the determination of cognitive dysfunction. |
