      目的 通过回顾性数据挖掘方法分析健脾温阳通络方新风胶囊(XFC)联合五味温通除痹胶囊(WWT)对类风湿关节炎(RA)寒湿痹阻证患者长链非编码RNA(lncRNA)MIR22HG、凝血纤溶指标及凋亡指标的影响。方法 收集30例健康者和100例XFC+WWT治疗前后RA寒湿痹阻证患者的外周血单个核细胞(PBMCs),检测MIR22HG的表达、凝血纤溶指标[血小板计数(PLT)、纤维蛋白原(FBG)、D-二聚体(D-dimer)、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间(TT)、血小板活化因子(PAF)、前列腺素I2(PGI2)]和凋亡指标[B细胞淋巴瘤2相关X蛋白(Bax)和B细胞淋巴瘤2(Bcl-2)],观察健脾温阳通络法,即新风胶囊(XFC)联合五味温通除痹胶囊(WWT)对其的影响。结果 (1)与健康对照组相比,RA寒湿组患者MIR22HG、PLT、FBG、D-dimer、PAF和Bax升高,PGI2和Bcl-2降低(P<0.05)。(2)Spearman相关性分析结果显示,MIR22HG与FBG、D-dimer、PLT、PGI2呈正相关,与APTT、Bax呈负相关。(3)关联规则结果显示,MIR22HG与PAF、Bax、D-dimer、FBG、Bcl-2、PLT和APTT的支持度大于45%、置信度均大于95%、提升度均大于1,具有关联性。(4)与治疗前相比,健脾温阳通络法治疗后RA寒湿痹阻证患者PGI2、Bcl-2升高,PLT、FBG、D-dimer、PAF、Bax和MIR22HG降低(P<0.05)。(5)关联规则结果显示,XFC+WWT与凝血纤溶指标的改善的支持度均大于45%,置信度均大于60%,提升度均大于1,有关联性。RA寒湿痹阻证患者治疗后PGI2、Bcl-2明显升高,PLT、FBG、D-dimer、PAF、Bax和MIR22HG降低。结论 RA寒湿痹阻证患者MIR22HG高表达并伴有凋亡和凝血纤溶指标的异常,健脾温阳通络法能提高RA寒湿痹阻证患者的MIR22HG表达水平,改善凋亡和凝血指标。

      Objective To review and summarize the effects of Jianpi Huashi Wenyang Tongluo Fang Xinfeng Capsule (XFC) combined with Wuwei Wentong Chubi Capsule (WWT) in patients with rheumatoid arthritis (RA) with cold-damp arthralgia syndrome lncRNA MIR22HG (MIR22HG) through retrospective data mining methods And the influence of coagulation,fibrinolysis and apoptosis indicators.Methods Peripheral blood mononuclear cells (PBMCs) from 30 healthy individuals and 100 patients with RA cold-dampness syndrome before and after XFC+WWT treatment were collected to detect the expression of MIR22HG,coagulation and fibrinolysis indexes [platelet count (PLT),fibrinogen (FBG),D-dimer,partial thromboplastin time (APTT),platelet activating factor (PAF),prostaglandin I2 (PGI2)] and apoptosis indicators [B-cell lymphoma -2 related X protein (Bax),B cell lymphoma-2 (Bcl-2)],observe the method of strengthening the spleen,warming yang and dredging collaterals,that is,the effect of Xinfeng capsule (XFC) combined with Wuwei Wentong Chubi Capsule (WWT) on it.Results (1) Compared with the normal group,PLT,FBG,D-dimer,PAF and Bax of 100 patients in the RA cold and damp group increased,and PGI2 and Bcl-2 decreased (P<0.05).(2) Correlation analysis results show that MIR22HG was positively correlated with FBG,D-dimer,PLT,PGI2,and negatively correlated with APTT and Bax.(3) The results of the association rules showed that MIR22HG supports PAF,Bax,D-dimer,FBG,Bcl-2,PLT and APTT with a degree of support of more than 45%,a confidence degree of more than 95%,and a degree of improvement.Greater than 1,with relevance.(4) Compared with before treatment,PGI2 and Bcl-2 of RA patients with cold-dampness block syndrome increased,and PLT,FBG,D-dimer,PAF,Bax and MIR22HG decreased (P<0.05).(5) The results of the association rules showed that the support for the improvement of XFC+WWT and coagulation and fibrinolysis indexes were both greater than 45%,the confidence is greater than 60%,and the promotion is greater than 1,indicating a correlation.PGI2 and Bcl-2 were significantly increased in RA patients with cold-dampness-blocking syndrome after treatment,while PLT,FBG,D-dimer,PAF,Bax and MIR22HG decreased.Conclusion The method of strengthening the spleen,dissipating dampness and warming yang and dredging collaterals can significantly improve the expression level of MIR22HG in RA patients with cold-dampness arthralgia syndrome and ameliorate apoptosis and coagulation.
