| 老年糖尿病患者是糖尿病人群中不可忽视的一大特殊群体,存在广泛的异质性,老年患者的临床管理超越了传统治疗脏器疾病的模式,是对多个领域的临床复杂性的评估及干预,老年综合评估(CGA)是老年科医生多年来为处理这种临床复杂性而开发的方法。CGA通过对一般情况、躯体功能状况、精神心理状况、共病、多重用药、认知功能、营养状况及社会支持程度的综合评估,可帮助为老年患者制定个体化的血糖控制目标,增强患者自我管理意识,优化药物治疗方案,制定合理的营养及运动计划,早期识别并发症及合并症。对于较长糖尿病病程、合并多重心血管危险因素或已有心血管病变的患者,个体化的适度管理目标和多靶点的综合管理是减少糖尿病相关死亡的必要策略。 |

| The elderly diabetic patients with extensive heterogeneity are a special group that cannot be ignored.The clinical management of elderly patients goes beyond the traditional model of treatment for organ diseases and it is an assessment and intervention of clinical complexity in multiple fields.Based on this,the comprehensive geriatric assessment (CGA) is a method developed by geriatricians for many years to deal with this clinical complexity.CGA can help to develop personalized control goals of blood glucose for elderly patients through comprehensive assessment of general conditions,physical function status,mental and psychological status,comorbidities,multiple medications,cognitive function,nutritional status and social support,also it can help them enhance the awareness of self-management,optimize drug treatment plans,formulate reasonable nutrition and exercise plans,identify complications and comorbidities early.For patients with a long course of diabetes,multiple cardiovascular risk factors or existing cardiovascular disease,personalized management goals and multi-target comprehensive management are necessary strategies to reduce diabetes-related deaths. |
