| 目的 探讨红细胞体积分布宽度(RDW-CV)在类风湿关节炎(RA)患者临床诊断中的价值。方法 选取2020年3月至2021年6月在安徽医科大学附属巢湖医院住院的76例RA患者为研究组；38例非RA关节病患者(强直性脊柱炎,反应性关节炎)为参照组；以及体检中心健康体检者33例为健康对照组。比较三组之间一般资料:红细胞(RBC)、血红蛋白(Hb)、RDW-CV、白细胞(WBC)、中性粒细胞百分比(N%)、淋巴细胞百分比(L%)、中性粒细胞与淋巴细胞比值(N/L)、血小板(PLT)、红细胞沉降率(ESR)；同时比较三组免疫学指标:抗环瓜酸肽抗体(抗CCP)、抗链球菌素“O”抗体(ASO)、类风湿因子(RF)、超敏C反应蛋白(hs-CRP)、免疫球蛋白G(IgG)、免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、补体C3(C3)、补体C4(C4)的差异；受试者工作特征(ROC)曲线分析RDW-CV在RA中的诊断效能；通过多元线性回归和Spearman相关分析探讨RDW-CV与其他指标的相关性。结果 三组间RBC、Hb水平:研究组低于参照组,且参照组低于对照组,差异均有统计学意义(P<0.05)；三组间RDW-CV水平:研究组高于参照组,且参照组高于对照组,差异均有统计学意义(P<0.05)；血清抗CCP抗体、ASO水平:研究组高于参照组,差异均有统计学意义(P<0.05)；血清C3、C4水平:研究组小于参照组和对照组,差异有统计学意义(P<0.05)。ROC曲线分析显示RDW-CV曲线下面积为0.728(P<0.05),其诊断RA的敏感度和特异度分别为63.20%、78.90%,诊断性能优于CRP和ESR；多元线性回归分析显示RDW-CV与RA病程、ESR、C3相关,回归方程决定系数0.341；相关性分析显示RDW-CV与RA病程正相关(r=0.352,P<0.05),与ESR正相关(r=0.313,P<0.05),与C3负相关(r=-0.232,P<0.05)。结论 RDW-CV可作为潜在的炎性指标,对RA诊断和病情判定具有一定的临床应用价值。 |

| Objective To investigate the clinical value of red blood cell volume distribution width(RDW-CV)in the diagnosis of rheumatoid arthritis (RA).Methods A total of 76 RA patients in Chaohu Hospital of Anhui Medical University from March 2020 to June 2021 were collected as case group,and 38 non-RA Rheumatic diseases (Ankylosing spondylitis,reactive arthritis) in the hospital during the same period were selected as the reference group,and 33 healthy volunteers from physical examination center were selected as healthy control group.The general data (RBC,Hb,RDW-CV,WBC,N%,L%,N/L,PLT,ESR) and immunological indicators (anti-CCP antibody,ASO,RF,hs-CRP,IgG,IgA,IgM,C3,C4) were measured.The diagnostic efficiency of each index was compared by ROC curve and the relationship between RDW-CV and other indexes was studied by Spearman correlation analysis and multiple linear regression.Results The level of RBC and Hb concentrations in the cases of RA were significantly lower than those of the reference and control (P<0.05).The level of RDW-CV in the cases of RA was significantly higher than that of the reference and control (P<0.05).Serum concentration of anti-CCP antibody and ASO in the patients of RA were the highest,followed by the reference and control consecutively (P<0.05).Serum concentrations of C3 and C4 were significantly lower than those of the reference and control (P<0.05).ROC curve analysis showed that the AUC value of RDW-CV in diagnosing of RA was 0.728,sensitivity was 63.20%,specificity was 78.90%,the diagnostic value was better than hsCRP and ESR.Multiple linear regression analysis showed that RDW-CV was correlated with ESR,C3,course of RA,and the standardization coefficient was 0.341 (P＜0.05).Spearman correlation analysis showed that RDW-CV was positively correlated with the course of RA (r=0.352,P<0.05),positively correlated with ESR (r=0.313,P<0.05),and negatively correlated which C3 (r=-0.232,P<0.05).Conclusions RDW-CV should be used as a potential inflammatory index,which has certain value in the diagnosis and condition evaluation of RA. |
