目的 探讨老年人尿失禁与衰弱的相关性。方法 回顾性分析473例住院老年患者的综合评估资料,尿失禁是采用症状诊断,衰弱是应用FRAIL量表筛查并测定躯体功能。应用多因素logistic回归探索尿失禁与衰弱的相关性。结果 身体强壮患者尿失禁的发生率是16.4%,衰弱前期患者尿失禁发生率为26.5%,而衰弱患者尿失禁的发生率为34.3%,各组间差异有统计学意义(χ2=12.15,P=0.002)。尿失禁患者较无尿失禁者躯体功能差,最大握力[(23.1±7.6)kg比(26.2±9.2)kg],步速[(0.85±0.37)m/s比(0.93±0.27)m/s],组间差异均有统计学意义(P值均<0.05)。多因素logistic回归分析提示衰弱是尿失禁的危险因素(OR:1.55,95%CI:1.15~2.10,P=0.004),躯体功能方面,步速与尿失禁相关(OR:0.36,95%CI:0.14~0.92,P=0.033)。结论 躯体功能尤其是步速与尿失禁相关,衰弱是老年人尿失禁的危险因素。 |

Objective To explore the correlation between urinary incontinence and frailty in the elderly.Methods The data of geriatric comprehensive assessment in 473 hospitalized elderly patients were retrospectively analyzed.Urinary incontinence was diagnosed by symptoms.Frailty was screened and measured by using the FRAIL scale.Multivariate logistic regression was used to explore the correlation between urinary incontinence and frailty.Results The incidence of urinary incontinence was 16.4% in the physically strong patients,26.5% in pre-frail patients,and 34.3% in frail patients,and the difference between the groups was statistically significant (χ2=12.15,P=0.002).Compared with the non-incontinence patients,the physical function of the incontinence patients was worse,the maximum grip strength[(23.1±7.6)kg vs.(26.2±9.2)kg]and the walking speed[(0.85±0.37)m/s vs.(0.93±0.27)m/s],and the differences between the groups were statistically significant (P<0.05).Logistic regression showed that frailty was a risk factor for urinary incontinence (OR:1.55,95%CI:1.15-2.10,P=0.004).In terms of physical function,walking speed was associated with urinary incontinence (OR:0.36,95%CI:0.14-0.92,P=0.033).Conclusions Physical function,especially walking speed,is associated with urinary incontinence.Frailty is a risk factor for urinary incontinence in the elderly. |
