      目的 探讨牙周病(PD)和年龄相关性黄斑变性(AMD)之间的关系。方法 从中国科学技术大学附属第一医院(安徽省立医院)健康管理中心数据库选取2018—2021年间新诊断的3 709例45岁以上AMD患者资料,对照组为3 709例45岁以上非AMD患者。对患者牙周病发生情况及人群高血压、外周动脉粥样硬化、糖尿病、慢性阻塞性肺疾病、肾功能不全发生情况、体重指数、三酰甘油、总胆固醇、高密度脂蛋白胆固醇水平等进行分析。采用单因素logistic回归分析AMD和PD及其他协变量之间的关系。采用多元logistic回归分析,排除混杂因素后,分析各组PD和AMD之间的相关性。结果 AMD组PD发生率(57.5%)高于非AMD组(39.0%),P<0.001。单因素logistic回归分析显示牙周病(OR=2.116,95%CI:1.928~2.321),高血压(OR=1.764,95%CI:1.609~1.934)和外周动脉粥样硬化(OR=1.278,95%CI:1.146~1.426)与AMD显著相关(P<0.001)。多因素logistic回归分析显示PD与AMD显著相关(OR=1.610,95%CI:1.02~2.54)。结论 牙周病可能是AMD的危险因素之一。
英文摘要:
      Objective To explore the association between periodontal disease and age-related macular degeneration related.Methods For this study,we enrolled adults at least 45 years of age from Anhui Provincial Hospital Health Management Center.The AMD group included 3 709 patients with newly diagnosed periodontitis between 2018 and 2021.The control group was frequency-matched by age and sex,with a sample size of 3 709 in each group.Logistic regression analysis was used to evaluate the associations between AMD and PD status and other covariables.In multivariate regression models,the overall analyses were adjusted for all confounding factors (hypertension,BMI,PAD,diabetes,serum HDL-C level) in order to examine the association between PD and AMD in each group.Results Univariate logistic regression analysis showed that presence of hypertension(OR=1.764,95%CI:1.609-1.934),PAD(OR=1.278, 95%CI:1.146-1.426)and PD(OR=2.116, 95%CI:1.928-2.321)were significantly associated with AMD in all participants.Multivariate logistic regression model after adjusting for all confounding factors showed that overall PD was significantly associated with AMD (OR=1.610,95%CI:1.02-2.54).Conclusions PD is independently associated with AMD in individuals.
