目的 考察乳腺不同组织经多层螺旋CT(MSCT)灌注成像检查中的血流灌注相关参数,分析其在评估乳腺癌微血管生成中的临床意义。方法 选取2018年1月至2019年12月河北北方学院附属第一医院98例成年女性单侧乳腺实质性肿瘤首诊患者,均经病理学检查结果确诊,其中乳腺癌70例(72个病灶),乳腺良性实质病变28例(65个病灶)。患者术前均行MSCT灌注成像扫描检查,获得感兴趣区(实质性乳腺肿瘤组织、癌旁正常乳腺腺体组织)的MSCT灌注参数,包括最大密度投影(MIP)、增强平均值(AV)、血流量(BF)、血容量(BV)、血管表面通透性(PS)、Patlak血容量(PBV)。对乳腺癌组织微血管密度(MVD)计数。比较实质性乳腺肿瘤组织、癌旁正常乳腺腺体组织的MSCT灌注参数,分析其与MVD的相关性。结果 乳腺癌组织MSCT灌注参数PS、MIP、AV、BF、BV、PS、PBV均值分别都高于乳腺良性实质肿块组织和乳腺癌旁正常乳腺组织,差异有统计学意义(P<0.05)。400倍镜下单个视野中乳腺癌组织的MVD为(24±7)个,乳腺良性实质肿块组织的MVD为(2±1)个,乳腺癌旁正常乳腺组织的MVD为(1±1)个,差异有统计学意义(F=503.051,P<0.001)。乳腺癌MSCT灌注成像参数中,MIP(r=0.42)、AV(r=0.44)、BF(r=0.49)、BV(r=0.47)、PS(r=0.48)、PBV(r=0.45)与MVD均呈正相关(P值均<0.05)。结论 临床应用MSCT灌注成像技术可以无创性在体诊断乳腺实质性肿块,较好地对乳腺癌肿瘤血管生成情况进行评估。 |

Objective To investigate the parameters related to blood perfusion in different tissues of breast by multi slice spiral CT perfusion imaging (MSCT),and to analyze their clinical significance in the evaluation of breast cancer microangiogenesis.Methods A total of 98 adult female patients with unilateral breast substantial tumor were selected from the First Affiliated Hospital of Hebei North University between January 2018 and December 2019.All of them were confirmed by pathological examination,including 70 cases of breast cancer (72 lesions) and 28 cases of benign parenchymal lesions (65 lesions).All patients underwent MSCT perfusion imaging scan before surgery,and MSCT perfusion parameters of the area of interest (substantive breast tumor tissue and normal breast gland tissue adjacent to cancer) were obtained,including:Maximum density projection (MIP),enhanced mean (AV),blood flow (BF),blood volume (BV),vascular surface permeability (PS),Patlak blood volume (PBV).The microvessel density (MVD) of breast cancer tissue was counted.MSCT perfusion parameters of substantive breast tumor tissue and paracancer normal breast gland tissue were compared to analyze the correlation between MVD and MSCT perfusion parameters.Results The mean values of PS,MIP,AV,BF,BV,PS and PBV of MSCT perfusion parameters in breast cancer tissue were higher than those in benign parenchymal mass tissue and normal breast tissue adjacent to breast cancer,respectively,with statistical significance (P<0.05).MVD of breast cancer tissue in a single field of vision at 400 times was 24±7,that of benign parenchymal mass tissue was 2±1,and that of normal breast tissue near breast cancer was 1±1,the difference was statistically significant (F=503.051,P<0.001).Among MSCT perfusion imaging parameters of breast cancer,MIP (r=0.42),AV (r=0.44),BF (r=0.49),BV (r=0.47),PS (r=0.48),PBV (r=0.45) were positively correlated with MVD (all P<0.05).Conclusions The clinical application of MSCT perfusion imaging technology can be used for noninvasive diagnosis of substantial breast masses,better evaluation of breast cancer tumor angiogenesis in vivo. |
