      目的 探讨2型糖尿病患者皮肤晚期糖基化终末产物(AGEs)的水平变化及其与糖尿病视网膜病变(DR)的关系。方法 选取2020年3月至2021年9月在蚌埠医学院第二附属医院内分泌科住院治疗的2型糖尿病患者110例,依据眼底检查结果,按有无视网膜病变分为无视网膜病变组(NDR组)36例、非增殖期视网膜病变组(NPDR组)40例、增殖期视网膜病变组(PDR组)34例;选取健康对照组(NC组)35例,分别测定皮肤AGEs和糖化血红蛋白(HbA1c)等指标。结果 与NC组相比,糖尿病组患者皮肤AGEs水平明显升高(P<0.01);与NDR组相比,NPDR组、PDR组皮肤AGEs明显增高(P<0.05),且PDR组皮肤AGEs水平明显高于NPDR组(P<0.05)。NPDR组HbA1c与NDR组、PDR组相比,差异有统计学意义。多因素logistic回归分析表明皮肤AGEs、糖尿病病程、HbA1c是DR的独立相关因素。结论 AGEs可能参与了DR的发病,皮肤无创检测AGEs对DR的诊治及预防有一定的指导意义。

      Objective To explore the levels of skin advanced glycation end products (AGEs) and its relationship with diabetic retinopathy (DR) in patients with type 2 diabetes.Methods One hundred and ten patients with type 2 diabetes who were admitted into the department of endocrinology and metabolism,the second affiliated hospital of Bengbu Medical College from March 2020 to September 2021 were selected.These patients were divided into three groups:no DR group (NDR group,n=36),non-proliferative DR group (NPDR group,n=40),proliferative DR group (PDR group,n=34) according to the results of fundus examination.35 healthy people from the outpatient medical examination center as the normal control (NC) group.The levels of skin AGEs and HbA1c,etc.were detected respectively.Results The levels of skin AGEs in diabetic patients were significantly increased compared with those in the NC group (P<0.01),the levels of skin AGEs in NPDR group and PDR group were significantly increased compared with those in the NDR group (P<0.05),the levels of skin AGEs in PDR group were significantly higher than those of the NPDR Group (P<0.05).The levels of HbA1c in NPDR group were significantly higher than those of NDR group and PDR group (P<0.05).The results of multivariate logistic regression analysis showed that skin AGEs,duration of diabetes and HbA1c were independent correlation factors of diabetic retinopathy (DR).Conclusions AGEs may be involved in the pathogenesis of DR,and non-invasive detection of AGEs in skin may be helpful in the diagnosis,treatment and prevention of DR.
