      目的 探讨2型糖尿病患者体成分与血脂异常的关系,为糖尿病患者的血脂管理提供临床证据。方法 回顾性分析2019年1月1日至2021年2月10日在武汉协和医院内分泌科住院治疗的2型糖尿病患者病历资料,分析实验室检查和人体成分测定结果,采用Spearman秩相关分析体成分与血清三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)的相关性,运用logistic回归分析体成分与血脂异常的关系。结果 本研究共纳入209例2型糖尿病患者,其中血脂异常人数126例(60.29%)。校正年龄和性别后,2型糖尿病患者TG与体重、体重指数、体脂肪量、体脂率、内脏脂肪面积、腰臀比呈正相关(P<0.05),HDL-C水平则与上述体成分指标呈负相关(P<0.05);TG和HLD-C与身体总水分、无机盐、蛋白质、肌肉量无显著相关性(P>0.05)。在亚组分析中,无论是否使用降脂治疗,TG和HDL-C与上述体成分指标的相关性总体趋势不变,但未使用降脂治疗的患者LDL-C与体脂肪量、体脂率、内脏脂肪面积、腰臀比呈正相关(P<0.05),而使用降脂治疗的患者LDL-C水平与上述指标无显著相关性(P>0.05)。校正年龄、性别、糖尿病病程、降脂药的使用情况后,2型糖尿病患者血脂异常的发生风险与体重(OR=1.04)、体重指数(OR=1.16)、体脂肪量(OR=1.06)、体脂率(OR=1.06)、内脏脂肪面积(OR=1.12)、腰臀比(OR=1.74)增加有关(P<0.05);但与总水分、蛋白质、无机盐、肌肉量无显著相关性(P>0.05)。结论 2型糖尿病患者体成分与血脂异常有关。改善体成分可能有利于2型糖尿病患者脂代谢异常的控制。

      Objective To explore the correlations between body composition and dyslipidemia in patients with type 2 diabetes mellitus (T2DM),and provide clinical evidence for lipid management in patients with T2DM.Methods Medical history data of patients with T2DM who were hospitalized in Department of Endocrinology,Wuhan Union Hospital from January 1,2019 to February 10,2021 were retrospectively analyzed.The results of laboratory examination and body composition analysis were analyzed.Spearman rank correlation analysis was used to analyze the correlations between body composition and serum triglycerides (TG),total cholesterol (TC),high-density lipoprotein cholesterol (HDL-C),and low-density lipoprotein cholesterol (LDL-C).Logistic regression was used to analyze the correlation between body composition and dyslipidemia.Results A total of 209 patients with T2DM were enrolled in this study,and there were 126 (60.29%) patients with dyslipidemia.After adjusting age and gender,the serum TG level was positively correlated body weight,body mass index (BMI),body fat mass (BFM),body fat percentage (BF%),visceral fat area (VFA) and waist-hip ratio (WHR) (P<0.05),while the HDL-C level was negatively correlated with above body composition indicators (P<0.05).There was no significant correlation between the serum TG and the HLD-C levels and total body water,mineral,protein and muscle mass (P>0.05).In the subgroup of lipid-lowering treatment,the correlations between the serum TG and HDL-C levels and the above body composition indicators remained unchanged regardless of whether lipid-lowering treatment was used.However,there was a positive correlations between serum LDL-C level and BFM,BF%,VFA and WHR in patients without lipid-lowering therapy (P<0.05).In patients treated with lipid-lowering therapy,the correlations between LDL-C level and above body composition indicators were no longer statistically significant (P>0.05).After adjusting age,gender,course duration of T2DM,and use of lipid-lowering drugs,the risk of dyslipidemia in patients with T2DM was associated with the increase of body weight (OR=1.04),BMI (OR=1.16),BFM (OR=1.06),BF% (OR=1.06),VFA (OR=1.12),and WHR (OR=1.74) (P<0.05);but it was not associated with total body water,mineral,protein and muscle mass (P>0.05).Conclusions The body composition is associated with dyslipidemia,improving body composition may be beneficial to the control of abnormal lipid metabolism in patients with T2DM.
