| 目的 探究60岁以上维持性血液透析(MHD)患者心理弹性分析及其与社会支持、应对方式及营养状况的相关性。方法 选取2020年6月至2021年6月于丽水市中心医院进行MHD的60岁以上患者282例,在治疗期间对患者进行心理弹性量表简表(RS-14)、社会支持量表(SSRS)、医学应对问卷(MCMQ)及营养不良-炎症评分(MIS)调查,比较不同弹性心理水平患者社会支持、应对方式及营养状况差异,并分析患者心理弹性与社会支持、应对方式及营养状况的相关性。结果 维持性血液透析患者RS-14总分为(69.07±7.33)分,其中心理弹性水平正常患者56例(A组),心理弹性水平降低患者226例(B组)；不同心理弹性水平的患者其SSRS总分、MCMQ评分及MIS评分比较,差异有统计学意义(P＜0.05)。经Pearson相关性分析得MHD患者心理弹性与SSRS量表中客观支持、主观支持、支持利用度及MCMQ量表中面对呈明显正相关(P＜0.05),与MCMQ量表中回避、屈服、MIS评分呈明显负相关(P＜0.05)。结论 MHD患者心理弹性总体呈轻中度降低水平,且与社会支持、应对方式及营养状况密切相关,在透析期间应鼓励患者主动寻求社会支持,积极面对疾病,加强自身营养支持,以增强心理弹性水平。 |

| Objective To explore the psychological resilience of maintenance hemodialysis (MHD) patients over 60 years old,and its correlation with social support,coping style and nutritional status.Methods A total of 282 patients over 60 years old who underwent MHD in the Lishui Central Hospital were selected between June,2020 and June,2021.The Resilience Scale (RS-14),Social Support Rating Scale (SSRS),Medical Coping Modes Questionnaire (MCMQ) and malnutrition-inflammation scale (MIS) were used for investigation.Social support,coping styles and nutritional status of patients with different psychological resilience levels were comparatively analyzed.The correlation of psychological resilience with social support,coping styles and nutritional status of patients was analyzed.Results The total score of RS-14 was (69.07±7.33).There were 56 patients with normal psychological resilience level (group A) and 226 patients with lower psychological resilience level (group B).There were statistically significant differences in SSRS scores,MCMQ scores and MIS scores between patients with different psychological resilience levels (P<0.05).Pearson correlation analysis showed that psychological resilience of MHD patients was significantly positively correlated with objective support,subjective support and support utilization in SSRS,as well as confrontation in MCMQ (P<0.05),but significantly negatively correlated with avoidance and surrender in MCMQ,and MIS score (P<0.05).Conclusions MHD patients have relatively lower psychological resilience level,which is closely related to social support,coping styles and nutritional status.During dialysis,patients should be encouraged to actively seek social support,actively face the disease and dialysis,and strengthen their own nutritional support so as to enhance psychological resilience. |
