      目的 探讨老年骨质疏松(OP)患者骨代谢指标与维生素D和维生素K水平的相关性。方法 选取2020年4月至2021年4月在重庆黔江民族医院收治的600例OP患者,采用随机数字表法选择同期进行 健康体检结果良好的200例健康者作为对照组。记录所有患者的临床病理特征,采用酶联免疫法(ELISA)检测 血清25羟基维生素D3(25-OH-VD3)和维生素K2(Vit K2)水平,采用全自动化学发光免疫分析仪检测血清I型 原胶原N-端前肽(PINP)、I型胶原交联C-末端肽(β-CTX)、骨钙素N端中分子片段(N-MID)和甲状旁腺素 (PTH)水平,Pearson相关性分析25-OH-VD3、Vit K2 与骨密度(BMD)、骨代谢标志物相关性。结果 老年OP组腰椎 BMD水平、血清25-OH-VD3、Vit K2水平低于对照组,血清PINP、β-CTX和N-MID水平高于对照组(P<0.05); 经Pearson相关分析显示,老年OP患者的BMD与25-OH-VD3、Vit K2水平呈正相关,与PINP、β-CTX水平呈负 相关(P<0.05);血清25-OH-VD3水平与Vit K2呈正相关,与PINP、β-CTX呈负相关(P<0.05);血清Vit K2水 平与PINP、β-CTX呈负相关(P<0.05)。结论 血清25-OH-VD3、Vit K2、PINP、β-CTX和N-MID水平的检测可 作为评估和治疗老年OP的依据。
英文摘要:
      Objective To analyze changes of bone metabolism indexes,vitamin D and K levels in elderly patients with osteoporosis (OP) and their correlation.Methods A total of 600 patients with OP were enrolled between April 2020 and April 2021.Other 200 healthy controls during the same period were randomly enrolled as control group.The clinicopathological characteristics of all patients were recorded.The levels of serum 25-OH-VD3 and Vit K2 were detected by enzyme-linked immunosorbent assay (ELISA).The levels of serum PINP,β-CTX,N-MID and PTH were detected by full-automatic chemiluminescence immunoassay.The correlation between 25-OH-VD3, Vit K2 and BMD,bone metabolism markers was analyzed by Pearson correlation analysis.Results The levels of lumbar BMD,serum 25-OH-VD3 and Vit K2 in OP group were significantly lower than those in control group (P<0.05),levels of serum PINP,β-CTX and N-MID were significantly higher than those in control group (P<0.05),but there was no significant difference in PTH level between the two groups (P>0.05).Pearson correlation analysis showed that BMD was positively correlated with 25-OH-VD3 and Vit K2 levels (P<0.05),negatively correlated with PINP andβ-CTX levels (P<0.05),but not significantly correlated with N-MID or PTH (P>0.05).The level of serum 25-OH-VD3 was positively correlated with Vit K2 level (P<0.05),negatively correlated with PINP and β-CTX levels (P<0.05),but not significantly correlated with N-MID or PTH (P>0.05).The level of serum Vit K2 was negatively correlated with PINP and β-CTX levels (P<0.05),but not significantly correlated with N-MID or PTH (P>0.05).Conclusion The detection of serum 25-OH-VD3,Vit K2 PINP,β-CTX and N-MID can be applied as clinical evaluation and treatment basis in elderly patient with OP.
