Insulin resistance is a state in which peripheral tissues do not respond adequately to insulin, gradually leading to impaired glucose tolerance and then to the development of type 2 diabetes. An increasing number of epidemiological studies have found that insulin resistance is closely related to the development and progression of intracranial and cervical vascular lesions. However, due to issues of measurement accuracy, study sample size, and confounding factors, there is not yet clear evidence to support that early screening for insulin resistance can prevent intracranial and cervical vascular disease.More accurate measurement tools and analytical strategies are still needed to properly interpret the causal relationship between insulin resistance and intracranial and cervical vascular lesions. This paper reviews the progress of research on the correlation between insulin resistance and intracranial and cervical vascular lesions, focusing on the epidemiology and measurement methods of insulin resistance, and its correlation with intracranial and cervical vascular lesions.
