| 目的 调查并分析老年患者肺部感染的营养状态,为营养治疗提供依据。方法 根据方便取样原则,选择2020年1月至2022年1月于首都医科大学附属北京友谊医院收治的200例老年肺部感染患者作为研究对象,分为≥80岁的高龄组(n=92)和<80岁的非高龄组(n=108)。采用简易营养评价精法(MNA-SF)评分评估老年肺部感染患者的营养状态,测量患者体重指数(BMI),检测血清白蛋白(Alb)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、血红蛋白(Hb)、总淋巴细胞计数(TLC)及C反应蛋白(CRP)水平,分析老年患者肺部感染的营养状态。结果 高龄组患者的TLC及Alb、Hb水平均低于非高龄组(P＜0.05),高龄组患者的CRP水平、淋巴细胞计数减低占比均高于非高龄组(P＜0.05)。200例老年肺部感染患者MNA-SF评分≥11分的患者有70例(35.00%),为营养良好组,<11分的有130例(65.00%),为营养不良组；营养不良组患者的淋巴细胞计数减少者占比、CRP水平均高于营养良好组(P＜0.05)。相关性分析结果显示,老年肺部感染患者MNA-SF评分与Alb、TC、Hb和TLC呈正相关性(P＜0.05),与年龄、LDL-C和CRP呈负相关性(P＜0.05)。结论 老年肺部感染患者的营养不良发生率较高,伴营养不良患者易出现免疫功能低下。 |

| Objective To investigate and analyze the relationship between pulmonary infection and nutritional status in elderly patients so as to provide basis for nutritional support intervention.Methods According to the principle of convenience sampling,200 elderly patients with pulmonary infection admitted to the hospital between January 2020 and January 2022 were enrolled as the research objects.They were divided into elderly group (≥80 years,n=92) and non-elderly group (<80 years,n=108).The nutritional status of elderly patients with pulmonary infection were evaluated by short-form mini-nutritional assessment (MNA-SF).The body mass index (BMI) was measured.The levels of albumin (Alb),total cholesterol (TC),high density lipoprotein cholesterol (HDL-C),low density lipoprotein cholesterol (LDL-C),hemoglobin (Hb),total lymphocyte count (TLC) and C-reactive protein (CRP) were detected.The relationship between pulmonary infection and nutritional status was analyzed.Results The levels of TLC,Alb and Hb in elderly group were lower than those in non-elderly group (P<0.05),CRP level and proportion of cases with immunodeficiency were higher than those in non-elderly group (P<0.05).Among the 200 elderly patients,there were 70 cases (35.00%) with MNA-SF score ≥11 points (good nutrition group) and 130 cases (65.00%)<11 points (malnutrition group).The times of pulmonary infection,proportion of cases with immunodeficiency and CRP level in malnutrition group were higher than those in good nutrition group (P<0.05).The results of correlation analysis showed that MNA-SF scorewas positively correlated with Alb,TC,Hb and TLC (P<0.05),while negatively correlated with age,LDL-C and CRP (P<0.05).Conclusions Elderly patients with pulmonary infection have a higher incidence of malnutrition,and malnourished patients are prone to immunocompromised. |
