| 目的 对氨基酮戊酸光动力治疗阴道上皮内瘤变(VaIN)的疗效和安全性进行初步评价。方法 收集2020年2月至2021年5月在中国科学技术大学附属第一医院南区行氨基酮戊酸光动力治疗的24例VaIN患者的临床资料,分析其治疗效果。结果 24例VaIN患者经6次氨基酮戊酸光动力治疗后,1个月阴道镜活检病理提示阴道壁病变治愈20例(83.3%),持续存在4例,进展0例；VaIN Ⅰ级病变治愈率85.7%(12/14),VaIN Ⅱ/Ⅲ级病变治愈率80.0%(8/10),差异无统计学意义(P＞0.05)。3个月复查人类乳头瘤病毒(HPV)转阴率63.6%(14/22,2例未愈的VaIN Ⅱ/Ⅲ更改治疗方案,未纳入统计),其中HPV 16/18转阴率60.0%(6/10),其他高危型HPV转阴率66.7%(8/12),差异无统计学意义(P＞0.05)。治疗过程中及治疗后患者均无明显不适,无阴道粘连等并发症发生。治愈病例随访6~18个月未见复发。结论 氨基酮戊酸光动力是治疗VaIN的安全有效方法,可降低其复发率。 |

| Objective To evaluate the efficacy and safety of aminolevulinic acid photodynamic therapy for vaginal intraepithelial neoplasia.Methods The clinical data of 24 patients with vaginal intraepithelial neoplasia (VaIN) treated with aminolevulinic acid photodynamic therapy in the South District of the First Affiliated Hospital of University of Science and Technology of China from February 2020 to May 2021 were collected and analyzed.Results The 24 patients underwent 6 times of aminolevulinic acid photodynamic therapy,colposcopically-guided punch biopsy were conducted one month later,the pathological results showed that 20 (83.3%)cases were cured,4 cases persisted and no progress cases;the cure rate of VaIN Ⅰ was 85.7%(12/14)and 80.0%(8/10)of VaIN Ⅱ/Ⅲ,and there was no significant difference between them (P>0.05).HPV test were coducted at 3 months,the results showed that 63.6% (14/22)cases were no longer infected with HPV,included 60.0%(6/10) in HPV 16 /18 group and 66.7%(8/12)in other group,and there were no significantly difference between the two groups(P>0.05).During the treatment,no patients complained noticeable discomfort,and and there were no complications such as vaginal adhesion.Cured patients were followed up for 6-18 months without recurrence.Conclusions Aminolevulinic acid photodynamic therapy is a safe and effective method for the treatment of vaginal intraepithelial neoplasia. |
