| 目的 探讨B超对甲状腺微小结节性质评定的临床价值。方法 选取2018年3月至2022年7月在黄山首康医院就诊,B超检查为可疑恶性甲状腺结节患者188例,行B超引导下甲状腺穿刺,穿刺组织行传统细胞学HE染色涂片及液基薄层细胞涂片,根据其病理学检查结果为依据,归纳患者的影像学结果,分析微小结节的影像学特征。结果 可疑恶性甲状腺结节的188例患者细胞学结果:良性结节63例,恶性结节125例。B超显示甲状腺结节≤1.0 cm比＞1.0 cm恶性风险大(P＜0.01)；甲状腺结节纵横比≥1比纵横比＜1恶性风险大(P＜0.01)；甲状腺细小分叶结节比结节边界欠清恶性风险大(P＜0.05)；甲状腺低回声或极低回声结节恶性风险较大(P＜0.01)；甲状腺结节有微钙化灶恶性风险较大(P＜0.05)。结论 B超检查评定甲状腺恶性结节,尤对甲状腺微小结节的恶性程度判断有一定的价值,借助症状、体征及影像学特征,基本达到诊断目的。 |

| Objective To explore the clinical value of B-ultrasound in the evaluation of small thyroid nodules.Methods From March 2018 to July 2022,188 patients with thyroid nodules suspected to be malignant by B-ultrasound examination in the Huangshan Shoukang Hospital of Anhui were selected.These patients underwent B-ultrasound guided thyroid puncture,and the puncture tissue was subjected to traditional cytological HE staining smear and thinprep cytology test.According to the pathological examination results,the imaging results of the patients were summarized and the imaging characteristics of malignant nodules were analyzed.Results The cytological results of 188 patients with suspected malignant thyroid nodules:63 benign nodules and 125 malignant nodules.B-ultrasound showed that the malignant risk of thyroid nodule size ≤1.0 cm was higher than that of ＞1.0 cm (P<0.01).Thyroid nodules with aspect ratio ≥1 had a higher malignant risk than those with aspect ratio<1 (P<0.01).Small lobulated thyroid nodules had a higher risk of malignancy than poorly demarcated thyroid nodules (P<0.05).Hypoechoic or very hypoechoic nodules had a higher risk of malignancy (P<0.01).Thyroid nodules with microcalcification had a higher risk of malignancy (P<0.05).Conclusions B-ultrasound is especially valuable to judge the malignant degree of small thyroid nodules,with the help of symptoms and signs and imaging characteristics,the purpose of diagnosis is basically achieved. |
