      目的 探讨肌少症与老年稳定期慢性阻塞性肺疾病(简称慢阻肺)患者炎症介质的相关性。方法 筛选出符合条件的老年稳定期慢阻肺患者220例,按照是否合并有肌少症分为研究组和对照组。检测所有患者的肺功能以及循环炎症介质白细胞介素(IL)-6、IL-10和肿瘤坏死因子(TNF)-α的水平。分析2组临床指标及炎症介质的差异。并分析研究组骨骼肌质量指数(简称肌量)与炎症介质等指标的相关性。结果 在老年稳定期慢阻肺患者中,肌少症发病率为35.00%。与对照组相比,研究组年龄更大,体重指数(BMI)及第1秒用力呼气容积占预计值的百分比(FEV1%)更低,IL-6、IL-10以及TNF-α水平更高,差异均有统计学意义(P<0.05)。在研究组中,肌量与年龄呈负相关(P<0.05),与BMI、FEV1%呈正相关(P<0.05),与IL-6、IL-10及TNF-α呈负相关(P<0.05)。结论 患有肌少症的老年稳定期慢阻肺患者肌量与年龄、FEV1%及炎症介质密切相关。

      Objective To analyze the correlation between sarcopenia and inflammatory mediators in elderly patients with stable chronic obstructive pulmonary disease(COPD).Methods Two hundred and twenty eligible elderly patients with stable COPD were selected and divided into study group and control group according to whether there was sarcopenia or not.Pulmonary function and levels of circulating inflammatory mediators interleukin (IL)-6,IL-10,and tumor necrosis factor (TNF)-α were measured in all patients.The differences of clinical indexes and inflammatory mediators between the two groups were analyzed.The correlation between skeletal muscle mass index and inflammatory mediators was analyzed.Results The incidence of sarcopenia in elderly patients with stable COPD was 35.00%.Compared with the control group,the study group was older and had lower body mass index(BMI) and forced expiratory volume in the first second percent predicted value(FEV1%).The levels of IL-6,IL-10 and TNF-α were higher,and the differences were statistically significant (P<0.05).In the study group,muscle mass was negatively correlated with age (P<0.05),positively correlated with BMI and FEV1% (P<0.05),and negatively correlated with IL-6,IL-10 and TNF-α (P<0.05).Conclusions Muscle mass is closely correlated with age,FEV1% and inflammatory mediators in elderly stable COPD patients with sarcopenia.
