目的 探究老年酒精使用障碍(AUD)患者认知功能障碍与脑部谷氨酸复合代谢物的相对浓度(Glx/Cr)值的相关性。方法 选取2021年1—12月在淮安市第三人民医院诊治的92例老年AUD患者为观察组,另按照年龄、性别适配原则社会招募无饮酒史的健康志愿者46例为对照组。采用简易智力状态检查量表(MMSE)、蒙特利尔认知评估量表(MoCA)评估所有入组对象认知功能,应用氢质子磁共振波谱技术(1H-MRS)测定脑部Glx/Cr值,并分析认知功能障碍与脑部Glx/Cr值的相关性。结果 观察组MMSE、MoCA评分显著低于对照组(P<0.05),Glx/Cr值显著高于对照组(P<0.05)。观察组患者饮酒史、平均每日饮酒量、平均每月饮酒天数均与酒精使用障碍筛查量表(AUDIT)评分、脑部Glx/Cr值呈正相关(P<0.001),脑部Glx/Cr值与AUDIT评分也呈正相关(P<0.001);MMSE、MoCA评分均与AUDIT评分、脑部Glx/Cr值呈负相关(P<0.001);观察组患者饮酒史、平均每日饮酒量、平均每月饮酒天数均与MMSE、MoCA评分呈负相关(P<0.001)。结论 老年AUD患者认知功能障碍程度与脑部Glx/Cr值呈负相关,脑部Glx/Cr值越高,认知功能障碍越严重。 |

Objective To explore the correlation between cognitive dysfunction and brain glutamate and glutamine/creatine (Glx/Cr) in elderly patients with alcohol use disorder (AUD).Methods A total of 92 elderly patients with AUD treated in the hospital were enrolled as observation group between January and December 2021,while 46 healthy volunteers with matched age and gender and without drinking history were enrolled as control group.The cognitive function of all objects was evaluated by Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA).The brain Glx/Cr was measured by proton magnetic resonance spectroscopy (1H-MRS).The correlation between cognitive dysfunction and brain Glx/Cr was analyzed.Results MMSE and MoCA scores in observation group were significantly lower than those in control group (P<0.05),and Glx/Cr value was significantly greater than that in control group (P<0.05).The drinking history,average daily alcohol consumption and average monthly days of drinking were positively correlated with AUDIT score and brain Glx/Cr value (P<0.001),brain Glx/Cr value was also positively correlated with AUDIT score (P<0.001),while MMSE and MoCA scores were negatively correlated with them (P<0.001).The drinking history,average daily alcohol consumption and average monthly days of drinking were negatively correlated with MMSE and MoCA scores (P<0.001).MoCA score was positively correlated with MMSE score (P<0.001).Conclusions The severity of cognitive dysfunction is negatively correlated with brain Glx/Cr value in elderly patients with AUD.The greater the brain Glx/Cr value,the more severe the cognitive dysfunction. |
