      目的 探讨中老年骨质疏松与β胶原特殊序列(β-CTX)/总1型胶原氨基端延长肽(T-P1NP)比值的相关性。方法 选取2019年1月1日至2021年5月31日在南京医科大学附属宿迁第一人民医院住院的中老年患者167例(男35例,女132 例),依据所检测的骨密度(BMD)结果,分为:骨量正常组(35例,男10例,女25例),骨量减少组(46例,男15例,女31例)和骨质疏松组(86例,男10例,女76例)。采用双能X线骨密度测定仪测定研究对象腰椎(BMD-L)、股骨颈(BMD-N)及髋关节全部(BMD-H)的骨密度;采用化学发光法测定血清T-P1NP和β-CTX,计算β-CTX/T-P1NP的比值。结果 骨量正常组β-CTX水平低于骨量减少组和骨质疏松组β-CTX水平(P=0.004);骨量正常组T-P1NP水平与骨量减少组和骨质疏松组T-P1NP水平比较,差异无统计学意义(P=0.247);β-CTX/T-P1NP比值,骨量正常组低于骨量减少组低于骨质疏松组,差异有统计学意义(P<0.001);骨密度(T值)与β-CTX/T-P1NP比值呈负相关,差异有统计学意义(P≤0.001)。结论β-CTX/T-P1NP比值对中老年骨质疏松症的早期诊断和疗效评估具有一定的参考价值。

      Objective To investigate the correlation between bone metabolism marker β-CTX /T-P1NP and osteoporosis in the middle-aged and elderly.Methods A total of 167 middle-aged and elderly patients (35 males and 132 females),admitted to the First People′s Hospital of Suqian Affiliated to Nanjing Medical University from January 1,2019 to May 31,2021 were selected.According to the results of BMD (BMD),they were divided into normal bone mass group (n=35,10 males,25 females),osteopenia group (n=46,15 males,31 females) and osteoporosis group (n=86,10 males,76 females).The BMD of lumbar spines (BMD-L),femoral neck (BMD-N) and total hip joint (BMD-H) of the subjects were measured by dual-energy X-ray BMD instrument.Serum bone metabolism markers such as T-P1NP and β-CTX were detected by electrochemiluminescence immunoassay,then the β-CTX /T-P1NP ratio was calculated.Results The three groups were compared each other.The level of serum β-CTX in normal bone mass group was lower than that in osteopenia group and osteoporosis group (P=0.004).There was no significant difference in T-P1NP level between normal bone mass group and osteopenia group and osteoporosis group (P=0.247).The β-CTX /T-P1NP ratio of normal bone mass group was lower than that of osteopenia group and lower than that of osteoporosis group (P<0.001).Bone mineral density (T-value) and β-CTX/T-P1NP ratio was negatively correlated,there was a statistical difference (P≤0.001).Conclusion The ratio of β-CTX /T-P1NP has certain reference value for early diagnosis and therapeutic evaluation of osteoporosis in middle-aged and elderly patients.
