      目的 探讨亚甲基四氢叶酸还原酶(MTHFR)和纤溶酶原激活剂抑制物-1(PAI-1)基因多态性与老年慢性肾衰竭(CRF)的相关性。方法 纳入2020年2月至2023年2月期间100例老年CRF患者作为CRF组,同时期老年健康体检者100例作为对照组,检测MTHFR、PAI-1基因多态性,比较CRF组与对照组MTHFR、PAI-1基因型与基因频率分布情况,以及CRF患者MTHFR、PAI-1不同基因型血尿素氮、血肌酐、肾小球滤过率,采用logistics回归分析MTHFR、PAI-1基因多态性与CRF患病的关系。结果 CRF组MTHFR C677T基因位点TT基因型、T等位基因占比及PAI-1 4G/5G基因位点4G/4G基因型、4G等位基因占比均高于对照组(P<0.05);MTHFR基因TT基因型的CRF患者尿素氮、肌酐水平均高于其他基因型(P<0.05),肾小球滤过率低于其他基因型(P<0.05);PAI-1基因4G/4G基因型的CRF患者尿素氮、肌酐水平均高于其他基因型(P<0.05),肾小球滤过率低于其他基因型(P<0.05);多因素logistics回归分析显示,MTHFR TT基因型、PAI-1 4G/4G基因型均为老年人CRF患病的独立危险因素(P<0.05)。结论 MTHFR C677T位点的TT基因型及PAI-1 4G/5G位点的4G/4G基因型均与老年CRF的发生相关。

      Objective To explore the correlation between gene polymorphisms of methylene tetrahydrofolate reductase (MTHFR) and plasminogen activator inhibitor-1 (PAI-1) and chronic renal failure (CRF) in elderly patients.Methods One hundred elderly patients with CRF from February 2020 to February 2023 were included in CRF group,and 100 elderly healthy subjects with physical examination during the same period were enrolled as control group.Gene polymorphisms of MTHFR and PAI-1 were detected,and genotypes and gene frequencies of MTHFR and PAI-1 between CRF group and control group as well as blood urea nitrogen,serum creatinine and glomerular filtration rate of CRF patients with different MTHFR and PAI-1 genotypes were compared.Logistics regression analysis was used to analyze the relationship between gene polymorphisms of MTHFR and PAI-1 and CRF.Results The proportions of TT genotype and T allele at C677T gene locus of MTHFR and the proportions of 4G/4G genotype and 4G allele at 4G/5G gene locus of PAI-1 in CRF group were higher than those in control group (P<0.05).The levels of serum urea nitrogen and creatinine of CRF patients with TT genotype of MTHFR gene were higher than those of CRF patients with other genotypes (P<0.05) while the glomerular filtration rate was lower than that of patients with other genotypes (P<0.05).The levels of urea nitrogen and creatinine in patients with 4G/4G genotype of PAI-1 gene were higher than those of patients with other genotypes (P<0.05) while the glomerular filtration rate was lower than that of patients with other genotypes (P<0.05).Logistics multivariate regression analysis showed that TT genotype of MTHFR and 4G/4G genotype of PAI-1 were independent risk factors for CRF in the elderly (P<0.05).Conclusion The TT genotype at C677T locus of MTHFR and the 4G/4G genotype at 4G/5G locus of PAI-1 are related to the occurrence of elderly CRF.
