| 目的 探讨祛风通络类中药封包对类风湿关节炎疾病活动与NOD样受体蛋白3(NLRP3)炎症小体的影响。方法 将2023年9月至2024年2月安徽中医药大学第一附属医院收治的60例风湿痹阻型类风湿关节炎患者按随机数字表法分成观察组和对照组,每组30例。对照组口服甲氨蝶呤加叶酸治疗,观察组在对照组基础上加祛风通络的中药封包,疗程均为2周。比较治疗前后中医证候积分,视觉模拟评分量表(VAS),医生整体评估(PGA),类风湿关节炎临床疾病活动指数(CDAI),美国风湿病学会类风湿关节炎20%、50%、70%改善率标准(ACR20/50/70)评价的临床疗效,28个关节疾病活动评分(DAS28),健康状况调查简表,焦虑自评量表(SAS)、抑郁自评量表；检测治疗前后超敏C反应蛋白、红细胞沉降率(ESR)、类风湿因子、抗环瓜氨酸肽抗体、NOD样受体蛋白3(NLRP3)、白细胞介素(IL)-6、IL-18、IL-1β、肿瘤坏死因子-α的水平。结果 治疗后,观察组较对照组在改善关节疼痛、关节压痛、晨僵方面效果显著；观察组ACR70改善率(60.0%)高于对照组(33.3%)；疾病活动度方面,观察组VAS、DAS28、CDAI、PGA评分均低于对照组(P＜0.05)；观察组在降低患者NLRP3、IL-6、IL-18,改善一般健康状况、生理功能、社会功能、躯体疼痛、心理健康、SAS等方面均优于对照组(P＜0.05)。结论 祛风通络类中药封包对类风湿关节炎患者临床疗效显著,能有效改善中医证候,降低疾病活动,降低炎症水平,提高患者生活质量。 |

| Objective To explore the effects of traditional Chinese medicine packaging with the method of Qufeng Tongluo on the disease activity of rheumatoid arthritis and NOD-like receptor protein 3 (NLRP3) inflammasome.Methods From September 2023 to February 2024,60 patients with rheumatoid arthritis of the rheumatoid arthritis type admitted to the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine were randomly divided into an observation group and a control group using a random number table method,with 30 patients in each group.The control group was treated with oral methotrexate and folic acid,while the observation group was treated with traditional Chinese medicine packets for dispelling wind and promoting collaterals on the basis of the control group.The treatment course was 2 weeks for both groups.Compared The traditional Chinese medicine syndrome scores,visual analog scale (VAS), physician′s global assessment (PGA),rheumatoid arthritis clinical disease activity index (CDAI),American College of Rheumatology rheumatoid arthritis 20%,50%,and 70% improvement rate criteria (ACR20/50/70) were compared to evaluate clinical efficacy before and after treatment,28 joint disease activity scores (DAS28),health status survey summary form,anxiety self-assessment scale (SAS),and depression self-assessment scale;the levels of high-sensitivity C-reactive protein,erythrocyte sedimentation rate (ESR),rheumatoid factor,anti cyclic citrullinated peptide antibody,NOD like receptor protein 3 (NLRP3),interleukin(IL)-6,IL-18,IL-1β,and tumor necrosis factor-α were detected before and after treatment.Results After treatment,the observation group showed significant improvement in joint pain,joint tenderness,and morning stiffness compared to the control group.The improvement rate of ACR70 in the observation group (60.0%) was higher than that in the control group (33.3%).In terms of disease activity,the VAS,DAS28,CDAI,and PGA scores in the observation group were lower than those in the control group (P<0.05).The observation group was superior to the control group in reducing patients′ NLRP3,IL-6,IL-18,improving general health status,physiological function,social function,physical pain,psychological health,SAS,and other aspects (P<0.05).Conclusions The traditional Chinese medicine package for Qufeng Tongluo has significant clinical efficacy in patients with rheumatoid arthritis.It can effectively improve traditional Chinese medicine syndromes,reduce disease activity,lower inflammation levels,improve life quality of patients with RA. |
